Getting to 10mg from 7.5mg Mounjaro: the clinical route explained

Why the titration schedule exists, and what SURMOUNT-1 actually showed

The four-weekly step-up pattern in the Mounjaro licensed schedule isn't arbitrary caution. SURMOUNT-1 enrolled 2,539 adults with obesity and no diabetes over 72 weeks, and participants who moved through doses steadily reported far better tolerability than those who escalated too quickly. At the highest doses, average body-weight reductions reached around 20–21%, figures cited by NICE in their appraisal of tirzepatide (TA1026). Those results depend on staying on treatment long enough to reach a maintenance dose, which is exactly why prescribers won't simply hand out 10mg to someone who hasn't yet settled on 7.5mg.

What this means practically: if you've been on 7.5mg for at least four weeks and tolerated it without significant nausea, vomiting or other persistent side effects, you're likely in the window where a prescriber would consider the step to 10mg appropriate. If side effects are still troublesome, the right move is usually to stay at the current dose a little longer, sometimes an extra two to four weeks makes a meaningful difference. That conversation belongs with your prescriber, not with a pharmacy checkout.

The only legal route to a 10mg pen in the UK

Tirzepatide is a Prescription-Only Medicine. That means a registered prescriber must assess your suitability and issue a prescription before any pharmacy can dispense it. There is no route that skips this step, and anyone offering 10mg Mounjaro pens without requiring a valid prescription is operating outside the law.

The MHRA has been clear about this risk. Following enforcement action that included the seizure of around 20 million doses of illegally traded weight-loss medicines worth roughly £45 million, and the raid of a counterfeit tirzepatide manufacturing site in Northampton in October 2025, the regulator continues to warn that pens bought through unverified social-media sellers or websites with suspiciously low prices carry real physical risk. Some fakes have contained substances entirely different from the labelled medicine. Report anything that looks off at the MHRA's Yellow Card service.

How the dose-increase process works at nume

Eligibility for 10mg Mounjaro: what a prescriber considers

Private eligibility for tirzepatide follows the licensed criteria: adults with a BMI of 30 or above, or a BMI of 27 or above alongside at least one weight-related condition such as high blood pressure, type 2 diabetes, high cholesterol, obstructive sleep apnoea or osteoarthritis. Lower BMI thresholds apply for some ethnic backgrounds under UK clinical guidance. But eligibility for the medicine is separate from eligibility for a specific dose. Reaching 10mg requires that the prescriber is satisfied 7.5mg was tolerated and that continuing upward serves your health safely.

Tirzepatide isn't recommended during pregnancy, while breastfeeding, or for anyone trying to conceive. It isn't licensed for people under 18. Certain medical histories (including a history of pancreatitis or medullary thyroid carcinoma) require careful prescriber discussion before any dose is issued.
