Getting Mounjaro on the NHS is possible, but the criteria are strict and the rollout is phased. As of mid-2026, you must have a BMI of at least 35 and four or more specific weight-related conditions to qualify under the current NHS England cohort — and even then, availability depends on your GP practice and local area. The NHS route, the private route, and how to navigate both safely

Step 1, Check whether the NHS criteria currently apply to you

The NHS England rollout of Mounjaro (tirzepatide) for weight management follows a phased schedule set by NICE technology appraisal TA1026. In the first cohort, now active, the threshold is a BMI of 40 or above alongside four or more of five qualifying conditions: type 2 diabetes, high blood pressure, dyslipidaemia, obstructive sleep apnoea, or cardiovascular disease. A second cohort (BMI 35 to 39.9 with four or more of those conditions) became eligible from 23 June 2026. Later phases will lower the bar further, with cohorts expected around March 2027. For some South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, thresholds are 2.5 kg/m² lower throughout.

Worth doing before your GP appointment, because knowing your number in advance keeps the conversation focused. Meeting the BMI threshold doesn't guarantee access, though. Your GP practice must participate in the new prescribing contract (introduced April 2026, but opt-in only), and you must take part in NHS wraparound diet and activity support as part of treatment. Waiting lists at specialist services (which handle Wegovy under a separate NICE pathway) are commonly long. Step 2, If you don't meet NHS criteria, understand the legal private route

The only lawful way to get Mounjaro outside the NHS is through a private prescription following a genuine clinical assessment. That assessment must be carried out or overseen by a registered prescriber who reviews your health, weight, medical history and any medicines you're already taking. There is no shortcut. The NHS's own guidance on tirzepatide confirms it is a prescription-only medicine requiring professional oversight.

Once a prescriber issues a private prescription, a GPhC-registered pharmacy can dispense it. The GPhC (General Pharmaceutical Council) is the UK regulator for pharmacies; every legitimate online pharmacy must be on its register. Step 3, Use the MHRA's warnings to spot illegitimate sellers

The MHRA has issued multiple public warnings about counterfeit and illegally sold weight-loss pens. In October 2025 enforcement teams raided the UK's first illicit manufacturing site producing fake tirzepatide pens, and across 2025 roughly 20 million doses with an estimated street value of around £45 million were seized. Some fake pens have contained insulin rather than the stated medicine, with serious consequences for patients. The MHRA's advice is consistent: buy only from a pharmacy you can verify on the GPhC register, and be wary of any seller offering these medicines without a prescription, through social media, or at prices far below what a legitimate private market charges. Discount codes or voucher offers for prescription-only medicines from unknown sellers are a red flag, not a saving. Suspect sellers can be reported through the MHRA Yellow Card scheme.
