Step 1, confirm the pen still has a dose left

A Mounjaro KwikPen carries four weekly doses. Each time you inject, the dose counter on the barrel decreases by one: 4, 3, 2, 1. When the counter shows '1', you have one full dose remaining, the golden dose. If the counter reads '0', the pen is empty and should go straight into a sharps container for safe disposal. Before you do anything else, check that number. It sounds obvious, but it is the step most people skip when they are in a hurry, and it is where most confusion about a missing dose begins.

Step 2, prepare the pen correctly before injecting

The golden dose injects in exactly the same way as the first three. Remove the pen cap, attach a fresh needle (if your pen uses a separate needle), and prime as directed in the Patient Information Leaflet supplied with your pen, a small droplet confirming the needle is clear. Rotate your injection site (abdomen, thigh or upper arm) away from your previous location; the skin needs time to recover between injections.

Temperature matters here. A pen that has been sitting in the fridge needs a few minutes at room temperature before you use it, injecting cold liquid is more uncomfortable than it needs to be. Step 3, inject the golden dose without losing any

Press the pen firmly against your skin at a 90-degree angle and hold the injection button down fully. This is the step where people sometimes let go too early. Keep the button depressed and hold the pen in place for a full ten seconds before removing it. The liquid is viscous, and the ten-second hold ensures the complete dose has been delivered rather than pooling under the skin and leaking back out when you withdraw the needle.

You may notice only a tiny amount of liquid remains in the cartridge after injecting, this is expected. A small overfill is built into the pen to ensure the labelled dose is delivered in full.
