How to Load a Wegovy Pen Before Your First Injection

Your pen is already loaded, here is what that actually means

Picture this: your Wegovy pen has just arrived, discreetly packaged, and you are standing at the fridge wondering which end to pull first. The reassuring news is that Novo Nordisk designed the pen so that the medicine is already inside. There is no vial to crack open, no syringe to fill, and no plunger to prime in the traditional sense.

Each pen comes factory-filled with enough semaglutide for four weekly injections at a set dose strength. The counter on the side of the pen shows the number of doses remaining, starting at four and counting down after each use. When the counter reaches zero, that pen is finished and you move to the next one. If you are just starting Wegovy, your first pen will be the 0.25 mg strength, a starting dose designed to let your system adjust gradually before the prescriber steps you up.

Attaching the needle: the step people most often miss

Needles are not pre-attached. Each weekly injection needs a brand-new sterile needle, fitted immediately before you inject and removed as soon as you are done. Leaving a needle on between uses lets air enter the pen and can affect dose accuracy, so it is a step worth building into your routine.

Take a needle from the pack, peel back the outer foil, and push the paper-covered inner cap straight onto the pen's needle thread, turning clockwise until it sits firmly. Pull off the outer needle cap and set it aside, you will need it to remove the needle safely afterwards. Then pull off the inner needle cap and discard it. Do not recap with the inner cap.

At this point you may notice a small drop of liquid at the needle tip, or you may not, either is normal. The pen does not require a traditional air-shot or priming step beyond what the manufacturer's leaflet specifies; follow that leaflet precisely rather than instructions from any other source. Checking the dose window and the dose counter before you inject

Before pressing the injection button, look at two things: the dose counter and the liquid in the small inspection window near the needle end of the pen. The liquid should be clear and colourless. If it looks cloudy, discoloured, or you can see particles, do not use that pen and contact your prescriber or our aftercare team.

The dose counter confirms how many doses the pen still holds. If it reads zero, the pen is empty even if you can still see a little liquid inside, that residual amount is not a usable dose. Move to a fresh pen.

One detail that catches people out on their first use: the counter does not move until the injection is complete and you have held the pen in place for the full count (typically six seconds, as described in the leaflet, always follow the leaflet for your specific pen). Lifting the pen away too early can mean an incomplete dose. Many people find it helps to count silently, especially on a busy morning before the school run when the instinct is to rush.

After the injection: needle removal, storage and what comes next

Once the pen clicks and you have held it in place, withdraw it from your skin and immediately recap the used needle using only the outer cap you set aside earlier. Twist it anticlockwise to remove it from the pen and place it directly into a sharps container. Never put a used needle in your household bin. A 1-litre sharps container is the right size for home use and is accepted by most local pharmacies and GP practices for safe disposal.

Return the pen (without a needle attached) to the fridge. The dose counter now shows one fewer dose than before. Rotate your injection site each week (abdomen, thigh, or upper arm are all suitable) to reduce the chance of skin irritation building up in one spot.
