What does the licensed Wegovy pen actually look like in practice?

Wegovy arrives as a pre-filled injection pen. The medicine is already inside, in the correct concentration, ready for subcutaneous injection into the abdomen, thigh or upper arm. There is no vial, no powder, no diluent, no syringe to draw up, and no cartridge to seat or prime in the way an insulin pen cartridge works. You attach a needle, perform an injection check, and inject. That is the whole process.

Each pen in the licensed titration schedule (from the starting dose through to the 2.4mg or 7.2mg maintenance) is individually sealed, refrigerated in the pharmacy and dispatched in cold-chain packaging. Is there any safe way to use an unlicensed semaglutide cartridge?

No. This is not a question of technique. The problem is not that mixing is difficult; it is that an unlicensed, compounded or counterfeit semaglutide product has not been assessed for purity, correct concentration, sterility or stability by the MHRA. The MHRA warned publicly in October 2023 about fake weight-loss pens seized in the UK, some of which were found to contain insulin. Injecting insulin when you intend to inject a GLP-1 medicine can cause dangerously low blood sugar.
