Start journey Learn moreTirzepatide 10mg, sold in the UK as Mounjaro 10mg, comes ready to inject in a pre-filled KwikPen. There is nothing to mix. The medicine is already dissolved in a clear solution inside the pen — you do not add water, reconstitute a powder, or draw up a volume. If you have seen instructions elsewhere about mixing tirzepatide, they are describing compounded or unlicensed preparations that are not the product Eli Lilly manufactures for the UK market. Mounjaro's KwikPen is a self-contained device: uncap, press against the injection site, press the button, and the dose is delivered. This page explains why the mixing question arises, what the 10mg pen actually contains, and what to check if your device or instructions seem unfamiliar — because in a market where counterfeit pens have been seized in significant quantities, an unexpected step in the process is worth pausing on. These are prescription-only medicines, and a GPhC-registered prescriber assesses every patient before treatment can begin.

The question is an understandable one. Semaglutide and other GLP-1 medicines are sometimes available in compounded form in countries where the licensed product is hard to obtain, and compounded versions do require reconstitution: you add sterile water to a lyophilised (freeze-dried) powder and mix before drawing up a dose in a syringe. That process has become well-documented online. People researching tirzepatide (particularly those who have seen US-based content) sometimes assume the same applies here.
It does not. In the UK, Mounjaro is only licensed as a pre-filled multi-dose KwikPen, manufactured by Eli Lilly under strict Good Manufacturing Practice conditions. No compounded tirzepatide product holds MHRA marketing authorisation. The NHS medicines page for tirzepatide describes a ready-to-use injection pen throughout, there is no mixing step anywhere in its guidance. If you have encountered a product that asks you to mix something before injecting, it is not UK-licensed Mounjaro.
The MHRA has made clear that GLP-1 medicines sold without a valid prescription through unverified channels carry serious risks, including the possibility of counterfeit or incorrectly labelled products. Large seizures of fake pens (some containing insulin rather than the labelled medicine) have taken place since 2023. A product that requires mixing is one of several red flags worth acting on immediately, and if you are curious about what a legitimate reconstitution process looks like by contrast, our guide on how to mix tirzepatide 5mg explains the steps involved and why they do not apply to the UK-licensed pen.
Each Mounjaro 10mg KwikPen holds 3 ml of solution, providing four individual weekly injections of 10mg tirzepatide. The solution is sterile, pre-measured, and ready to administer. Before use, you inspect it visually: the liquid should appear clear and colourless to faintly yellow, with no floating particles or cloudiness. If it looks otherwise, the pen should not be used.
The injection sites are the abdomen, front of the thigh, or upper arm. Rotate the site each week. For precise guidance on needle attachment, priming (the pen is supplied primed, but your patient information leaflet confirms this) and injection technique, the leaflet inside the box is the definitive source. Doses and timing, including whether 10mg is the right step for you, are decisions your prescriber makes based on how your previous dose was tolerated. There is more on what the 10mg dose means clinically on the 10mg tirzepatide dosage page.
Storage matters too. The pens are refrigerated at 2–8°C. There is a limited window during which they can be kept at room temperature, but that window varies and the exact figure belongs in the leaflet, not in general copy, the SmPC on the Electronic Medicines Compendium is the right reference for storage windows.
The injection technique is identical across all Mounjaro strengths. The KwikPen design is consistent, the same cap, the same button, the same visual inspection process. What changes is the concentration of tirzepatide in the solution, not anything about how you handle or administer it. You do not adjust volumes or unit measurements; the pen delivers the correct dose automatically when you press the button fully and hold it for the count specified in your leaflet.
Patients who have been on Mounjaro 5mg and are moving to 10mg sometimes wonder whether anything changes practically. The answer is: the device looks and works the same. The prescriber's decision to increase the dose is what shifts, and that decision follows a clinical review, not a patient's self-assessment. If you are thinking about dose adjustment or have questions about where you are in the titration schedule, the right conversation is with your prescriber rather than with a dosing calculator.
If you want to understand how the 10mg dose translates into the unit measurements sometimes referenced in compounding contexts, the 10mg tirzepatide in units page addresses that directly, though it also makes clear why those measurements do not apply to the Eli Lilly pen.
Start with where it came from. Mounjaro is a prescription-only medicine in the UK. It should reach you from a pharmacy you can verify on the GPhC register, the GPhC entry for nume's pharmacy is public, and you can check any online pharmacy the same way at pharmacyregulation.org. A legitimate pen arrives in Lilly's original outer carton, with a batch number, expiry date, and a patient information leaflet printed in English. The KwikPen itself has the strength printed clearly; the cap fits snugly; the solution window lets you inspect the liquid before use.
Red flags include: a pen or packaging that looks slightly different from the leaflet photographs, instructions to mix or reconstitute the product, a seller who provided it without a clinical assessment, and prices far below the market range. The guide to buying Mounjaro online safely covers each of these in more detail. If anything about your pen raises a doubt, do not inject. Contact your pharmacist, and if you suspect a counterfeit, report it to the MHRA via the Yellow Card scheme, the reporting form takes under five minutes and helps protect other patients.
Getting the prescription right matters as much as the pen itself. At nume, a real clinician (a GPhC-registered Independent Prescriber) reviews every consultation personally before any treatment is dispatched. You can check your eligibility and start a free consultation if you are considering treatment for the first time or thinking about transferring from another provider.
