How to store your Mounjaro injection correctly

Fridge, drawer or travel bag, which matters most?

The choice that trips up most people is not whether to refrigerate the pen (that part is usually clear) but where inside the fridge to put it, and what to do when they are away from home. Mounjaro should sit on a middle shelf, away from the back wall where coils can cause localised freezing, and away from the door where every opening brings a temperature spike. Keeping it in its original cardboard carton does two jobs at once: it shields the medicine from light and gives the pen a stable little home that is easy to move safely.

Freezing is an irreversible problem. If your pen has been frozen (even briefly) the medicine is no longer usable. The NHS tirzepatide medicines page is clear on this point, and the same guidance applies whether the freezing happened in a fridge set too cold, an ice pack pressed directly against the pen, or a cold car overnight in January.

Light degradation is a quieter risk. Ultraviolet exposure can break down the active ingredient over time, which is why the carton exists. A clear pen left on a sunny windowsill or beside a lamp for an extended period may look identical but perform differently. Keep it boxed.

Room temperature: what the rules actually say

A question our prescribers hear most weeks is some version of: "I left my pen out overnight, is it ruined?" The honest answer depends on how long and how warm. Mounjaro pens can tolerate room temperature up to 30°C for a defined window, after which the pen should be used or discarded. That window is printed in your Patient Information Leaflet, and the Mounjaro SmPC on the eMC is the authoritative source if you want to verify it independently, we deliberately do not state the exact number of hours here because updated SmPC versions take precedence over any website. What you should never do is pop the pen back in the fridge after it has been kept at room temperature to "reset the clock", once it has been out, the clock keeps running.

For travel, a purpose-made insulin cooling wallet or a gel-pack cooler (not a hard ice pack in direct contact with the pen) keeps it within range. If you are going abroad, check airline rules on carrying injectable medicines in hand luggage, and bring a copy of your prescription.

How long does a pen stay usable, and when should you act on that?

The Mounjaro injection is delivered via a KwikPen designed for four weekly doses. The question of how long Mounjaro can be stored has a different answer depending on whether you are talking about an unopened pen (the expiry date on the carton, within the cold chain) or a pen you have already started (the four-week in-use period, which does not reset).

A pen that has passed its expiry date should not be used, even if it looks fine and has been kept cold throughout. Similarly, if you notice any cloudiness, particles or discolouration when you inspect the cartridge window before injecting, do not use it, the liquid should be clear and colourless. If you are ever in doubt, your prescriber or pharmacist is the right person to ask rather than guessing.

Disposal: the part people forget until the bin is full

Used needles are classified as clinical waste and cannot go in a household recycling bin or general rubbish. The standard approach is a dedicated sharps container, a rigid, puncture-resistant yellow box that your local council or GP surgery can help you dispose of safely. If you need one, a sharps container is available from our pharmacy alongside your treatment.

The pen body itself, once all four doses have been used, goes in your household rubbish (not recycling), but the needle should always be removed and placed in the sharps bin immediately after each injection. Never recap a used needle. This applies regardless of whether you are storing the pen in the fridge for another three uses or finishing your last dose of the pen.

Mounjaro is a prescription-only medicine, and safe disposal is part of the clinical responsibility that comes with using it.
