The biggest myth about 5mg Mounjaro: that it works the same way as the starter pen

A question our prescribers hear most weeks goes something like this: "I'm on 5mg now, do I just do exactly the same thing as I did on 2.5mg?" Mostly yes, but there is a distinction worth understanding. The 2.5mg pen exists primarily to let your body adjust to tirzepatide, keeping side effects manageable while your system gets used to the medicine. At 5mg you move into the therapeutic range, and the medicine's appetite-reducing effect typically becomes more noticeable. Expecting nothing to change is the misconception. Step-by-step: injecting from the 5mg KwikPen correctly

The Mounjaro KwikPen is designed to make self-injection straightforward. Before you start, wash your hands thoroughly. Take the pen from the fridge and allow it to reach room temperature for a few minutes, cold solution can sting more. Remove the base cap and check the solution window: it should be clear or slightly yellow; if it looks cloudy or contains visible particles, do not use it and contact your prescriber or our support team.

Choose your injection site, abdomen (at least 5 cm from the navel), front of the thigh, or the back of the upper arm if someone is helping you. Pinch a fold of skin if needed. Press the pen firmly against your skin, then press the button and hold it down until you hear two clicks and the indicator window changes, then hold for five more seconds to make sure the full dose is delivered. Dispose of the pen in a sharps container after use; do not recap or reuse needles.

Rotate your injection site every week. Using the same spot repeatedly causes lipohypertrophy, a fatty lump under the skin that slows absorption and reduces how well the medicine works. The Mounjaro SmPC on the eMC sets out the full injection instructions as approved by the MHRA.

Side effects at 5mg and when to get medical help

The most common side effects at this dose are gastrointestinal: nausea, loose stools, constipation, burping and indigestion tend to head the list. They are usually most pronounced in the first few days after stepping up to 5mg and fade over the following week or two for most people. Eating smaller portions, choosing lower-fat foods and keeping well hydrated all help the body adapt. Fatigue and mild headache are also reported, particularly in the early weeks.

A small number of people experience more serious effects that need prompt attention. Severe stomach pain that persists and spreads towards the back, with or without vomiting, can signal pancreatitis and requires urgent medical assessment, the MHRA specifically highlighted this risk in a January 2026 Drug Safety Update on GLP-1 medicines. Symptoms of gallbladder problems (sharp right-upper abdominal pain, especially after eating fatty food), signs of a serious allergic reaction (swelling of the face, difficulty breathing), or significant dehydration from persistent vomiting or diarrhoea all warrant a call to 111 or your GP without delay.

Staying on track: dose day, missed doses and what comes after 5mg

Pick a day of the week that works reliably for you and stick to it. The once-weekly schedule is flexible about the time of day, morning, evening, with a meal or without, all are fine. What matters is the seven-day gap. If you miss a dose and there are still four or more days before your next scheduled injection, take it as soon as you remember. If it is fewer than four days away, skip it and return to your usual day. Never take two doses in the same week to make up for a missed one. After four weeks on 5mg your prescriber will consider whether to increase to 7.5mg. That decision depends on how you have tolerated 5mg and, in time, on evidence of weight loss.
