How to use your Mounjaro pen a second time (and what most people get wrong)

The myth: you need a brand-new pen each week

This is the most common point of confusion our prescribers hear. Many people assume the Mounjaro pen is single-use (one pen, one injection) and feel anxious when told to put it back in the fridge. That anxiety is completely understandable; you've just done something new and slightly nerve-racking, and the idea of keeping a partially used medical device for another week feels unfamiliar.

The reality is straightforward. Each KwikPen comes pre-filled with enough tirzepatide for four weekly doses. After your second Mounjaro injection, and the third, and the fourth, the process is the same every time. The pen itself is not exhausted, only the needle you attached for that injection is finished. Remove it, dispose of it safely, cap the pen and return it to the fridge. That is all.

What you do differently on the second injection compared with the first

Functionally, very little changes. The main practical differences are the injection site and the needle.

First, choose a different spot from where you injected the week before. Rotating between the abdomen, thigh and upper arm prevents skin irritation from building up at a single site. If you used the left side of your abdomen first, the right side or a thigh works well next. Second (and this is the step that matters most for safety) attach a completely new needle before you inject. Needles should not be left on the pen between doses. Leaving a needle attached can allow air or fluid to enter the cartridge and may affect the dose. Reusing a needle risks blunting (which makes injection more uncomfortable) and infection. After attaching the new needle, the dose indicator should already be set from your prescriber's instructions. Do not change it unless your prescriber has told you to. Inject slowly into a clean, pinched area of skin, and hold for ten seconds before withdrawing. Clean the skin beforehand with an alcohol swab and let it dry before inserting the needle.

One more thing: the day and time you inject can stay the same each week. Consistency helps you remember and keeps the interval between doses even.

Storing and handling the pen between doses

Once the needle is removed and the pen is capped, it goes straight back into the fridge at 2–8°C. The pen does not need to be kept upright or away from light in any particular orientation beyond what your leaflet specifies. Do not freeze it.

If the pen has been out of the fridge (for travel, for example) there is a limited window during which it remains usable at room temperature. The exact duration is stated in the Patient Information Leaflet and the Mounjaro SmPC on the eMC; that is the definitive source, not a number quoted online. Do not guess: if you are unsure whether your pen is still viable after time out of the fridge, contact your prescriber or our clinical team before injecting.

Used needles must go into a sharps container immediately after each injection, never into household bins. A one-litre sharps container is the right size for home use with a four-dose pen.

When to speak to a prescriber rather than pressing ahead

There are situations where using the pen again should wait until you have clinical input. If the dose window looks different from what it should be, if there is a visible change in the liquid (cloudiness, particles, discolouration), or if the pen mechanism does not feel right, do not inject. Equally, if you missed last week's dose and are unsure whether to double up or continue as normal: the answer is never to double up, but timing advice for your specific situation belongs with your prescriber, not a webpage.

The most common side effects (nausea, loose stools, burping, a reduced appetite) are typically noticed in the first day or two after an injection and tend to ease as the weeks go on. If side effects feel unusually severe, or if you develop persistent stomach pain that reaches through to your back, seek medical attention and report it to the MHRA Yellow Card scheme.
