It is not listed as a common side effect in the Wegovy prescribing information, but there are real, well-understood reasons it can happen — mostly tied to eating less and losing weight rather than to semaglutide directly. Many people starting Wegovy assume that any unfamiliar sensation must be a direct drug side effect. Feeling cold is a good example of a symptom that gets attributed to semaglutide when the mechanism is usually something else entirely. The NHS patient information for semaglutide lists the recognised common side effects clearly, nausea, vomiting, diarrhoea, constipation, stomach pain, headache and fatigue among them. Feeling cold does not appear on that list.
That does not mean your experience is imaginary or unimportant. It means the likely explanation sits one step back from the medicine itself. Understanding that distinction matters, because it changes what you do about it.
Wegovy works by reducing appetite significantly. Most people on it eat considerably less, often without consciously trying. The body uses food partly as fuel for heat generation, a process called thermogenesis. Eat less, and your core temperature regulation shifts a little. Add in early fat loss (fat tissue acts as insulation), and feeling chillier than usual starts to make sense. It is the same reason people who have lost a lot of weight through any method often say they feel the cold more.
There is also a simpler explanation worth checking first: are you drinking enough? Dehydration affects circulation and can make extremities feel cold. GI side effects in the early weeks of Wegovy (particularly nausea or diarrhoea) can reduce fluid intake without you realising.

The headline pattern is gastrointestinal: most people experience some degree of nausea, especially after a dose increase, and it usually settles within a week or two as the body adjusts. Fatigue is also common in the early weeks, and tiredness and feeling cold can overlap in how they feel. One side effect that is less common but genuinely serious is pancreatitis. In January 2026 the MHRA issued a Drug Safety Update specifically highlighting acute pancreatitis as a known risk with GLP-1 medicines including semaglutide. The warning symptom is severe, persistent stomach pain that may spread to the back, sometimes with vomiting. That is not the dull background nausea most people notice, it is a sharper, escalating pain. If that describes what you are feeling, stop treatment and get medical help the same day. Gallbladder problems are another recognised side effect. Rapid weight loss in general increases gallstone risk, and Wegovy accelerates weight loss for most people. Right-sided or upper abdominal pain, particularly after eating, is worth reporting to your GP.
Cold hands and feet that come and go, or a general chilliness since starting treatment, are worth mentioning at your next clinical check-in but rarely need urgent action. There are situations, though, where cold-related symptoms deserve prompt attention.
If your hands or feet are consistently cold, numb or changing colour (white, then blue, then red), that can indicate Raynaud's phenomenon or a circulatory issue that is unrelated to Wegovy but should be assessed by a GP. If you feel cold alongside unusual fatigue, hair thinning and puffy skin, thyroid function is worth checking, again, not a Wegovy effect, but significant weight changes can sometimes unmask an underlying thyroid condition.
Feeling cold alongside severe nausea and an inability to keep fluids down for more than 24 hours points to dehydration, which can affect kidney function. The NHS semaglutide page notes that dehydration from GI side effects is something to take seriously. Hydrate steadily with small sips; if you cannot keep water down, contact your GP or call 111.
Any persistent or worrying symptom can also be reported to the MHRA Yellow Card scheme. You do not need a confirmed link between the symptom and the medicine to report, that is the point of the scheme.
First, keep a simple note of when it happens. Is it worse after eating (or not eating)? Tied to a particular time of day? Getting better or worse as your dose settles? That kind of pattern is useful information for your prescriber.
Second, check your fluid intake. A useful British habit: a glass of water before the kettle goes on each morning adds up over a week. Small, regular sips work better than large amounts at once if nausea is a factor.
Third, make sure you are eating enough protein and not dropping calories so sharply that your body is compensating by dialling down heat production. This is a genuine risk when appetite suppression is very effective. A prescriber or dietitian can help you structure meals that work alongside the medicine rather than against it.
