Leftover Wegovy in your pen: what it means and what to do next

Step 1, Understand why there is medicine left in the pen

The Wegovy pen is a pre-filled, multi-dose device. It is not designed to be emptied in a single sitting. Each pen delivers one subcutaneous injection per week for four weeks, and the volume remaining after each injection is simply the medicine for your next scheduled dose. That visible liquid in the cartridge window is yours to use, nothing has gone wrong.

A common misconception worth clearing up gently: some people assume they should inject until the pen feels empty. That is not how it works. The pen's dose mechanism measures and delivers a precise amount each time; you do not control or top up the volume manually. When the pen is truly finished after four uses, the plunger will have reached its end stop and the pen should be disposed of in a sharps container.

Step 2, Store the open pen correctly between injections

Once a Wegovy pen has been used for the first time, storage becomes critical. The in-use pen should be kept refrigerated at 2–8 °C when not in use. The Patient Information Leaflet states a maximum of 56 days from first use for a refrigerated pen, after that point, the remaining doses should not be used even if liquid is still visible.

Room-temperature storage is permitted for a limited period, but the exact window is stated in the leaflet and on the Wegovy Summary of Product Characteristics on the eMC, not here, packaging and guidance do change, so always read the current leaflet. Keep the pen cap on between uses to protect the needle tip. Do not freeze the pen, and keep it away from direct light and heat sources.

Step 3, Decide whether the remaining doses are still usable

Before each injection, run through a short mental checklist. First, count the days since you first used the pen: if it is beyond 56 days, dispose of it. Second, check the liquid through the cartridge window, it should be clear and colourless; do not use it if it appears cloudy, discoloured or contains particles. Third, confirm the pen has been stored within the correct temperature range throughout.

Step 4, Dispose of finished or unusable pens safely

A pen that has reached its 56-day limit, shows visible changes to the liquid, or has run out of doses should be disposed of as clinical waste. Putting it in household recycling or a general bin is not safe. A purpose-made sharps container is the correct route: seal the full container and return it to a pharmacy or local authority collection point.
