Semaglutide at a Medspa: Evidence, Licensing and the Safer UK Route

The trial data that brought semaglutide into the spotlight

The STEP 1 study enrolled 1,961 adults with obesity or excess weight alongside a weight-related condition, and ran for 68 weeks. Participants taking semaglutide 2.4mg weekly alongside a reduced-calorie diet and increased activity lost an average of around 15% of body weight, compared with around 2.4% in the placebo group. NICE's recommendation (TA875) has specific conditions attached: use within a specialist weight management service, for a maximum of two years, in adults with a BMI of 35 or above alongside at least one weight-related comorbidity. Lower BMI thresholds apply for certain ethnic backgrounds. That clinical framework matters because the trial results came alongside proper dose titration, monitoring and lifestyle support, not a standing injection at a beauty counter.

In April 2026, the MHRA approved a single-dose 7.2mg Wegovy pen, with trials reporting around 20.7% average weight loss over 72 weeks at that maintenance dose. Why the medspa setting raises specific questions in the UK

The medspa model (often combining aesthetic treatments with weight-management injections) is common in the United States and has started appearing in the UK. There is nothing inherently wrong with a clinic offering semaglutide alongside other services. The question is always the same: is there a qualified prescriber conducting a genuine clinical assessment, and is the medicine sourced through the licensed UK supply chain?

Semaglutide is a prescription-only medicine under UK law. That means a GPhC-registered or GMC-registered prescriber must review your medical history, weight, current medicines and any contraindications before a prescription is issued, wherever you are sitting when they do it. A medspa that employs, or has a formal arrangement with, a registered prescriber can operate perfectly legally. One that hands out injections with no clinical review cannot, and the medicine it supplies may not have come through legitimate channels.

The MHRA has repeatedly warned about counterfeit GLP-1 pens circulating outside the regulated supply chain, fake pens have, in documented cases, contained insulin rather than semaglutide. Checking that any clinic or pharmacy can be verified on the GPhC register is the single most important step before you commit to treatment anywhere. Eligibility: who semaglutide is licensed for in the UK

The private licence for Wegovy covers adults with a BMI of 30 or above, or a BMI from 27 upwards if at least one weight-related condition is present, for example, high blood pressure, type 2 diabetes, high cholesterol or obstructive sleep apnoea. BMI thresholds are 2.5 kg/m² lower for some ethnic backgrounds under UK guidance.

Those numbers are the starting point, not an automatic green light. Side effects and what to watch for

The most frequently reported effects are gastrointestinal: nausea, loose stools, constipation, indigestion and fatigue top the list in the trial data. They tend to cluster in the first few weeks of treatment and after each dose increase, then ease as the body adjusts. Staying hydrated and not skipping meals helps most people through that early phase.

Less common but more serious: the MHRA has highlighted acute pancreatitis as a known, infrequent risk with GLP-1 medicines.
