Picture the scenario: your new pen arrived in a plain box with DPD tracking, looks identical to the ones before it, and the dose wheel now reads 10mg. You inject as usual. For some people, nothing much changes, the nausea they had at 7.5mg has already settled, and 10mg barely registers. For others, the step up brings a familiar but unwelcome return of queasiness in the first two to four days, sometimes accompanied by looser stools or a sluggish digestion that swings between constipation and urgency.
This is the typical pattern with tirzepatide 10mg side effects. Mounjaro slows how quickly food leaves the stomach, and the body needs time to recalibrate each time the dose increases. Eating smaller portions, choosing lower-fat meals and staying well hydrated all help. Fatigue and mild headache are also reported at this stage, mostly in the first few days. Burping and reflux can accompany the slowed gastric emptying. Injection-site redness or itching remains possible at any strength; rotating sites between the abdomen, thigh and upper arm helps. Most of what people experience at 10mg is uncomfortable rather than dangerous. But there is a small group of side effects that require you to seek medical help promptly, and it's worth knowing them clearly before you start the pen.
Pancreatitis is the most important one to understand. In January 2026, the MHRA issued a Drug Safety Update confirming acute pancreatitis as a known, infrequent but serious risk with GLP-1 medicines including tirzepatide. The symptom to act on is severe stomach pain that persists and may spread through to the back, with or without vomiting. This is not the ordinary post-injection nausea that fades by lunchtime, it is a pain that doesn't let up. If you experience it, stop eating and drinking and get medical attention the same day.
Gallbladder problems are also associated with significant weight loss and GLP-1 treatment: sudden pain in the upper right abdomen, particularly after eating, is worth reporting to a doctor. Signs of dehydration (extreme thirst, very dark urine, dizziness) can develop if severe vomiting or diarrhoea is not managed; staying hydrated matters, and if you can't keep fluids down, that is a reason to seek help rather than wait it out.
Allergic reactions (rash, swelling of the face or throat, difficulty breathing) and sudden changes in vision should prompt urgent medical attention. The threshold for contacting a clinician should be lower than most people set it. You don't need a textbook emergency to reach out. If nausea at 10mg is bad enough that you're struggling to eat or drink for more than a day or two, that's worth a conversation. If you've lost significantly more weight than expected in a short period and feel weak, your prescriber should know. If something just feels different or wrong compared with your previous doses, say so.

The 10mg dose sits in the middle of the licensed range. Treatment begins at 2.5mg (a tolerability step whose job is to let the body adjust before any meaningful dose is reached) and the licensed maximum is 15mg. Side effects at the lower end of the schedule, covered on our 2.5mg page, tend to be the most noticeable for many people simply because the GI system is encountering tirzepatide for the first time. By 10mg, the body has usually adapted to the mechanism, though the dose-increase effect can still bring a short adjustment period.
