What the SURMOUNT-1 data actually showed at 15mg

The SURMOUNT-1 trial randomised 2,539 adults with obesity (none of whom had type 2 diabetes) across three tirzepatide doses and a placebo group, over 72 weeks. At 15mg, participants lost an average of roughly 20–21% of their starting body weight. Reaching 15mg: how the titration schedule works

Tirzepatide treatment in the UK begins at 2.5mg — a dose calibrated to help your system adjust, not one intended to produce therapeutic weight loss on its own. The prescriber then steps the dose up, typically every four weeks, through 5mg, 7.5mg, 10mg and 12.5mg before reaching 15mg. That schedule exists because the most common side effects (nausea, loose stools, indigestion) tend to peak after a dose change and settle within days to a couple of weeks; a gradual approach makes them considerably more manageable.

Side effects to be aware of at the highest dose

Gastrointestinal effects are the most commonly reported across all tirzepatide doses, and 15mg is no exception. Nausea, vomiting, diarrhoea, constipation and reflux can occur, most often in the days after a dose change rather than as a persistent feature of treatment. Most people find these effects manageable and self-limiting, but they are more pronounced at the start of each new level than when the dose has been stable for several weeks.

Less common but important: the MHRA Drug Safety Update index includes a January 2026 communication on GLP-1 medicines highlighting acute pancreatitis as a known infrequent risk. Severe stomach pain that radiates to the back, particularly with vomiting, warrants urgent medical attention, stop the injection and call 111 or go to A&E. Gallbladder symptoms, signs of dehydration after persistent vomiting or diarrhoea, and injection-site reactions are also documented. Who reaches 15mg, and what a prescriber considers

The licensed population for Mounjaro includes adults with a BMI of 30 or above, or 27 and above if at least one weight-related condition is present (such as high blood pressure, type 2 diabetes, high cholesterol or obstructive sleep apnoea). Lower BMI thresholds apply for some ethnic backgrounds under UK guidance. Those are the conditions for starting tirzepatide, reaching 15mg specifically depends on how a person has responded to and tolerated the preceding doses.

NICE's guidance notes that if a patient loses less than 5% of their weight after six months at the highest tolerated dose, continuing should be reviewed. That threshold is a clinical prompt, not an automatic cut-off, and a prescriber considers the full picture: overall health, any co-existing conditions, how side effects have been managed and what the person's goals are.
