Start journey Learn moreMounjaro (tirzepatide) does not prevent exercise, but it does change the conditions you exercise in. Fatigue, nausea and dizziness are among the most commonly reported side effects during the early weeks of treatment, and physical activity can amplify how noticeable they feel. Understanding which symptoms are expected, which call for a pause, and which need urgent attention is the practical knowledge this page covers. These are prescription-only medicines; a GPhC-registered prescriber reviews suitability and side effects as part of every consultation at nume's free clinical consultation.

The SURMOUNT-1 trial (which enrolled 2,539 adults over 72 weeks) established that gastrointestinal effects are tirzepatide's most common side effects, affecting a significant proportion of participants, especially in the dose-escalation phase. Nausea, vomiting, diarrhoea and constipation were most frequent after starting treatment or stepping up a dose, and typically settled within days to around two weeks. The NHS tirzepatide patient information confirms this GI-led profile.
Exercise adds a meaningful variable. Vigorous activity diverts blood flow to working muscles, slowing gastric emptying further, which is already reduced by tirzepatide. Many people find nausea feels sharper during a run or a hard gym session in the first month of treatment than it would at rest. That is not a sign something has gone wrong; it is a predictable overlap of two processes acting on the gut at the same time. Adjusting the timing and intensity of exercise is usually enough to manage it.
One quick habit worth building: before any session, rate your energy and stomach comfort on a simple 1-to-5 scale. If both are below 3, a walk or a rest day is a reasonable call. It takes under a minute and stops you pushing into a session that turns a mild symptom into a miserable one.
Fatigue is listed as a common side effect in the Mounjaro prescribing information, and it can sit alongside a reduced calorie intake, which is part of how tirzepatide supports weight loss. The combination of eating less and adjusting to a new medicine means your body has less ready fuel than it used to. A session that felt routine before treatment may feel considerably harder in weeks two to six.
Dizziness, though less frequently reported than nausea, is also documented. It can occur on standing quickly after a floor exercise, or during prolonged cardio in warm conditions. If dizziness is happening during exercise, the sensible response is to stop, sit, hydrate and see whether it resolves within a few minutes. Persistent or severe dizziness is a reason to contact a clinician rather than push through.
There is also a consideration specific to active people: when calorie intake drops sharply on tirzepatide, protein adequacy matters for preserving muscle mass. This is a discussion for your prescriber or a dietitian rather than something to manage solo. Our clinical team page (led by an Independent Prescriber) outlines how clinical oversight shapes every stage of treatment at nume.
Most exercise-related side effects on tirzepatide are uncomfortable rather than dangerous, and they improve with time. The following warrant stopping activity immediately and seeking urgent care.
Severe, persistent abdominal pain that radiates to the back, with or without vomiting, during or after exercise is a possible sign of acute pancreatitis. In January 2026, the MHRA's Drug Safety Update specifically highlighted acute pancreatitis as a known but infrequent GLP-1 medicine risk that can be serious; this is not the ordinary stomach discomfort that settles after a rest. Seek emergency help.
Other red flags during exercise: signs of serious dehydration (no urine output, very dark urine, confusion, rapid heartbeat) following vomiting or diarrhoea; allergic reaction symptoms such as swelling of the face, lips or throat, difficulty breathing or severe skin reaction; and chest pain or shortness of breath disproportionate to exertion. These need emergency attention, not a wait-and-see approach.
For symptoms that are troubling but not emergencies, the full guide to Mounjaro risks and side effects covers the wider picture. You can also review the if-side-effects page for practical management steps. And if a side effect feels serious or unexpected, report it via Yellow Card, the link is in the safety facts panel above.
Clinical trials consistently show that tirzepatide works best alongside a reduced-calorie diet and increased physical activity; the UK licence says so explicitly. Exercise is not optional decoration on this treatment, it is part of the intended programme. The question is how to do it sensibly when side effects are active.
A few things that tend to help in practice: scheduling exercise at least two hours after eating, when gastric emptying has progressed; starting with lower-intensity activity (walking, swimming, light cycling) in the first month rather than returning immediately to previous training loads; and staying well hydrated before and during sessions, especially if GI symptoms have been present recently.
Resistance training is worth keeping in any exercise plan during GLP-1 treatment. With appetite reduced, maintaining muscle mass takes deliberate effort. If you want a broader picture of how Mounjaro works as a weight loss treatment, including how the dose-escalation schedule fits into a wider programme, that detail is covered in our main treatment guide. Any change to your training plan, especially if you are managing a health condition or have had significant side effects, is worth discussing with your prescriber. If you are noticing stomach-related symptoms during workouts, it helps to understand how Mounjaro side effects typically develop and resolve over the course of treatment, so you can judge whether what you are experiencing is expected. Some people experience very few side effects and can maintain their usual training from the start; individual responses vary considerably.
If you have questions about starting or continuing Mounjaro alongside a training programme, speak to our prescribers through a free consultation, suitability, side effects and lifestyle factors are all part of what they assess. It is also worth knowing that there are five Mounjaro side effects that come up most often for people who are physically active, and being aware of them in advance can help you plan your sessions more confidently.
