Getting Mounjaro in Cardiff: the real-world route

Step 1: understand which route is actually open to you right now

The SmPC criteria for private Mounjaro prescribing are broader than the NHS ones. Adults with a BMI of 30 or above qualify on BMI alone; those with a BMI of 27 to 29.9 qualify if they have at least one weight-related condition such as type 2 diabetes, high blood pressure, high cholesterol or obstructive sleep apnoea. For some ethnic backgrounds (including South Asian, Chinese, Middle Eastern, Black African and African-Caribbean backgrounds) UK guidance recognises lower BMI thresholds as clinically equivalent. BMI is the starting point, not the whole story. A prescriber also considers your medical history, any medicines you take, and conditions that would make Mounjaro unsuitable. History of medullary thyroid carcinoma, certain pancreatic conditions, pregnancy, breastfeeding, or trying to conceive are among the contraindications. Step 3: have your consultation and get treatment dispatched

Through a regulated online pharmacy, the process is entirely remote. You complete a clinical questionnaire, provide photo ID, and verify your weight on a short video call. Step 4: know what to expect once treatment begins

Mounjaro is a once-weekly injection, each pre-filled KwikPen providing four doses. Treatment starts at 2.5mg (a dose that exists to help your system adjust rather than to produce maximum effect) and is titrated by the prescriber, typically in four-week steps, up through 5mg, 7.5mg and beyond as tolerated. The most common side effects are gastrointestinal: nausea, loose stools, indigestion and reduced appetite. Most people find these are most noticeable in the first week or two after starting or increasing a dose, then settle.

If you take an oral contraceptive pill, the NHS advises adding a non-oral method of contraception for the first four weeks of treatment and for four weeks after each dose increase, because tirzepatide's effect on gastric emptying can reduce pill absorption.
