Mounjaro and Hair Loss: What Causes It and What to Do

Hair shedding linked to Mounjaro is usually telogen effluvium (a temporary stress response to rapid weight loss) rather than direct drug toxicity.
Nutritional factors during treatment (particularly low protein or iron intake) can make shedding worse and are worth reviewing with your prescriber.
In most people the shedding slows naturally within three to six months as weight stabilises; it does not typically lead to permanent hair loss.
If you experience sudden or severe hair loss, or any other unexpected change, tell your prescriber and consider reporting it via the MHRA Yellow Card scheme.

Hair shedding during or after Mounjaro treatment is real, and it is more common than the prescribing information suggests at first glance. The most likely cause is not the medicine itself but a well-documented biological response called telogen effluvium — the body's reaction to rapid weight loss and reduced calorie intake. Most people who notice it find the shedding slows on its own within a few months. Mounjaro (tirzepatide) is a prescription-only medicine, and any concerns about side effects, including hair changes, are best raised with your prescriber as part of your ongoing care. The NHS medicines page for tirzepatide lists commonly reported side effects and is a good starting point for understanding what the clinical evidence shows.

Understanding the hair loss question — so you can decide what to do next

Is Mounjaro itself causing the hair loss, or is something else happening?

This is the question at the centre of most conversations our prescribers have with patients who notice increased shedding. The honest answer is: it is probably both, and the distinction matters for what you do next.

Telogen effluvium is a recognised, well-studied phenomenon in which a physical stressor (significant weight loss, surgery, illness or a dramatic shift in calorie intake) pushes a larger-than-normal proportion of hair follicles from the active growth phase into the resting phase simultaneously. Around two to three months later, those follicles shed, often in noticeable clumps in the shower or on the pillow. Because Mounjaro suppresses appetite significantly and rapid weight loss follows, the body can register that change as a stressor. If you are wondering whether Mounjaro causes hair loss directly or whether it is the weight-loss process driving the shedding, the distinction matters more than most people realise. The hair loss reflects what's happening to your body weight, not a toxin in the pen.

Tirzepatide is not listed as a cause of alopecia in the formal SmPC side-effect profile, but patient-reported experiences and post-marketing data have been widely noted by clinicians. If you want to look at the full list of reported effects, the Mounjaro SmPC on the eMC is the authoritative reference. Our overview of Mounjaro side effects covers the broader profile in plain language.

What actually causes hair loss on Mounjaro, the three factors worth knowing

Knowing what causes hair loss on Mounjaro helps you assess your own situation rather than simply hoping it resolves. Three factors tend to work together.

First, calorie deficit. Mounjaro reduces appetite substantially. Some people, particularly in the early weeks, eat far less than their body needs to maintain normal follicle cycling. Hair is not an essential tissue, so when the body is rationing resources, follicles are deprioritised.

Second, protein intake. Keratin (the structural protein hair is made of) requires adequate dietary protein. On a markedly reduced appetite it is easy to get enough calories from carbohydrate-dense convenience foods while quietly under-eating protein. This compounds the shedding. A prescriber or dietitian can help you review your intake without disrupting your treatment.

Third, micronutrient status. Iron, ferritin, zinc and biotin are all associated with hair cycling. Rapid weight loss can affect absorption and levels. A blood test to check ferritin and iron is a reasonable step if shedding is significant.

Our dedicated page on Mounjaro and hair loss goes deeper on the nutritional side of this, and what the clinical data on tirzepatide and hair loss shows is explored separately for those who want the evidence detail.

When does hair shedding actually need medical attention?

Most shedding triggered by telogen effluvium does not require urgent action. It follows a predictable pattern: it typically starts two to four months after the trigger (the weight-loss phase), peaks over a few weeks, then gradually slows over the next three to six months as weight stabilises and nutritional intake recovers. It does not cause bald patches and does not lead to permanent follicle damage.

However, some presentations are a prompt to contact your prescriber or GP sooner rather than later. Seek medical advice if you notice patchy hair loss, loss from your eyebrows or body hair, loss that is accelerating rather than slowing, or shedding that began before any significant weight change. These patterns suggest a cause unrelated to telogen effluvium and need proper assessment. Conditions such as thyroid dysfunction, autoimmune alopecia and iron-deficiency anaemia can coincide with starting a new medicine and be wrongly attributed to it.

For anything unexpected during treatment, the MHRA Yellow Card scheme lets you report suspected side effects directly, this is how post-marketing safety data on medicines like Mounjaro is built and monitored. It takes about five minutes and is open to patients as well as clinicians. You can read more about Mounjaro generally on our main Mounjaro treatment page.

What you can actually do about it, practical steps and the consultation conversation

There is no proven treatment for telogen effluvium specifically, but there is plenty you can do to give your follicles the best conditions to recover while continuing Mounjaro.

Prioritise protein at every meal, even if your appetite is low. A palm-sized portion of chicken, fish, eggs, legumes or Greek yoghurt with each eating occasion is a practical target, think of it the same way you might think about hitting five a day. Some people find eating earlier in the morning useful; if the idea of food before 9am feels impossible right now, even a high-protein snack mid-morning counts.

Ask your prescriber to check ferritin, iron and thyroid function if you haven't had them checked recently. These blood tests are straightforward and can rule out an underlying cause that would need its own treatment.

Avoid crash eating patterns. The slower the weight loss, the less pronounced the telogen effluvium response tends to be. This is one reason the Mounjaro titration schedule exists, the dose increases gradually, giving your body time to adapt. There is no clinical case for rushing to a higher dose to speed up results if your current dose is working steadily.

If you have specific concerns about how tirzepatide is affecting you, our prescribers discuss side effects and overall suitability as part of every consultation. You can start a free consultation to speak to our clinical team about your situation. The connection between Mounjaro side effects and hair loss is a topic our team addresses regularly, and you will not be the first person to raise it.
