The SURMOUNT-1 trial, published in the New England Journal of Medicine, tracked over 2,500 adults taking tirzepatide across 72 weeks. The most common reasons people reduced or stopped treatment were gastrointestinal: nausea, diarrhoea, reduced appetite, and vomiting. These are documented in the NHS medicines page for tirzepatide and in the medicine's SmPC. None of them are trivial for hydration. When nausea makes eating uncomfortable or appetite drops sharply, fluid intake often falls alongside food intake, not because people forget to drink, but because the cues that prompt drinking (mealtimes, hunger, thirst reinforced by eating) are blunted.
Tirzepatide also slows gastric emptying, which is part of how it works. The stomach holds food for longer, which sustains fullness but also means the signals that normally prompt the next meal or drink come later. Over days or weeks, a modest but cumulative fluid deficit can develop. This is not unique to Mounjaro; it has been observed with GLP-1 medicines more broadly. The difference here is that tirzepatide's dual GIP and GLP-1 mechanism tends to produce stronger appetite suppression, so the effect on eating and drinking patterns can be more pronounced.
None of this means chapped lips are inevitable or that something is going wrong with your treatment. It means paying deliberate attention to hydration, something many people on tirzepatide find they need to actively build back in as a habit.
The most useful thing is straightforward: drink water steadily across the day, aiming for around 1.5–2 litres, without waiting until you feel thirsty. On Mounjaro, thirst cues can be less reliable than usual. Setting a reminder or keeping a bottle visible tends to work better than relying on the feeling itself.
Lip balm helps symptomatically, but it is treating the surface rather than the cause. If you are using it constantly and still finding your lips are cracking, that is a reasonable prompt to check whether your daily fluid and food intake has dropped further than you realised. Protein and healthy fats in particular matter for skin and mucosal health; very low calorie intake over several weeks can deplete the building blocks your body uses to maintain skin tissue.
Some people on tirzepatide also find that mouth breathing increases, partly because a slightly dry mouth and throat make nasal breathing less comfortable. If that is your experience, nasal strips or a humidifier at night can help, and are worth mentioning to your prescriber. Dose increases are the periods to watch most closely. Being proactive with hydration in those weeks reduces the chance of lip dryness becoming more than a minor nuisance. Mild lip dryness that responds to lip balm and improved fluid intake is not a clinical concern. There are signs, though, that point toward dehydration significant enough to need attention. Dark or infrequent urine, dizziness on standing, persistent headache, muscle cramps, or a noticeably dry mouth that does not improve with drinking are all worth reporting to your prescriber or GP promptly. These symptoms together suggest your body is not getting enough fluid to function well, and that can become a medical issue quickly, particularly if you are also experiencing vomiting or diarrhoea.
The MHRA's Yellow Card scheme exists so that patients can report any suspected side effect from a medicine, including unexpected skin changes, directly to the regulator. You can do that at yellowcard.mhra.gov.uk. It is worth knowing about even if you never need it.

It rarely requires dose adjustment.
