Practical steps that can reduce how rough day 2 feels

There are several things within your control today. Eating smaller portions (even if appetite is low) tends to help more than skipping meals entirely, which can worsen nausea. Plain, easy-to-digest foods (dry toast, crackers, rice, boiled potato) sit better than fatty or spicy meals; this is precisely the period when high-fat food interacts most noticeably with slowed gastric emptying.

Hydration matters more than it sounds. GI symptoms can lead to fluid loss, and even mild dehydration sharpens headaches and fatigue. Sipping water steadily across the day (rather than drinking large amounts at once) works better while nausea is present. Hot drinks are fine if tolerated.

Rest helps too. Fatigue on day 2 is common and tends to be most pronounced in the first injection cycle. If you can manage it, this is not the day to push a hard workout.

When to get medical help, and what counts as urgent

Most day 2 symptoms are uncomfortable rather than dangerous. But some signs need prompt attention, and it is worth being clear about which is which.

Call NHS 111 or seek medical help the same day if you experience severe, persistent stomach pain that spreads toward your back, with or without vomiting. This pattern can be associated with acute pancreatitis. Severe abdominal pain on day 2 is not typical nausea. Trust that distinction.

Also seek prompt help for signs of a serious allergic reaction (swelling of the face, throat or tongue; difficulty breathing; a widespread rash), and for gallbladder-related symptoms such as severe upper-right abdominal pain. If vomiting or diarrhoea is severe enough that you cannot keep fluids down for several hours, that too warrants a call to 111, because dehydration can put pressure on your kidneys.

For everything else (the low-grade nausea, the tiredness, the loose stools) watchful waiting over the next day or two is appropriate, with the practical steps above.
