This one comes up regularly. The belief is that some online sellers offer Mounjaro without a real assessment, or that a prescription is a technicality you can sidestep with the right search. It is not. Under UK law, tirzepatide cannot be legally supplied to any patient without a prescription issued by a registered prescriber following a genuine clinical review. The Human Medicines Regulations 2012 are unambiguous on this, and the MHRA has taken enforcement action against sellers operating outside these rules, including the seizure of approximately 20 million doses of illegally traded weight-loss medicines worth an estimated £45 million.
Sellers who offer these pens with no clinical assessment are either operating illegally or supplying counterfeit stock. The MHRA has explicitly warned that counterfeit pens have contained the wrong active ingredient (in some cases insulin) leading to hospitalisations. If you come across a source that does not require a completed health assessment and prescriber sign-off, that is the red flag, not a convenience. You can report suspect sellers directly at the MHRA Yellow Card service.
It is also worth understanding whether you can obtain Mounjaro without involving a GP at all, the answer is more nuanced than most people expect, and the rules differ between the NHS and the private route.
A question our prescribers hear most weeks is whether the prescription has to come specifically from a GP, or whether other qualified clinicians can issue one. In the UK, tirzepatide can be prescribed by any registered Independent Prescriber, which includes GPs, but also nurse prescribers and pharmacist prescribers who hold that qualification. What that assessment must cover: your BMI, your weight-related health conditions, your current medicines, relevant medical history, and identity verification. The licensed criteria for Mounjaro in the UK set a starting point of BMI 30 or above, or BMI 27 or above if you have at least one weight-related condition such as high blood pressure, type 2 diabetes, high cholesterol, or obstructive sleep apnoea. Lower BMI thresholds apply for some ethnic backgrounds under UK clinical guidance. Meeting the numbers is necessary but not sufficient, the prescriber must also consider your current medicines, any contraindications (a personal or family history of medullary thyroid carcinoma, for instance, rules out treatment), and whether tirzepatide is appropriate for you specifically.
This is why the prescription cannot be automated. Two people with the same BMI may get different outcomes from a legitimate assessment. That individual clinical judgment is exactly what protects you, and it is what separates a regulated prescription from a pen bought through an unverified source.
