What BMI do you need for Mounjaro in the UK?

The licensed starting point for Mounjaro eligibility in the UK is a BMI of 30 or above. If your BMI sits between 27 and 29.9, you can still qualify, provided you have at least one weight-related health condition. Conditions that count include type 2 diabetes, high blood pressure (hypertension), raised cholesterol (dyslipidaemia), obstructive sleep apnoea, and cardiovascular disease, among others. Ethnic background matters here. UK clinical guidance reduces the qualifying BMI threshold by 2.5 kg/m² for people of South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds. So a BMI of 27.5 can meet the weight-management threshold where it would otherwise be 30. This adjustment reflects evidence that body-fat distribution and metabolic risk differ across populations at the same BMI reading.

Does NHS eligibility for Mounjaro work the same way?

No, and this is a source of real confusion. Private eligibility follows the licensed indication in Mounjaro's SmPC: broadly, BMI 30 or above (or 27 with a qualifying condition). NHS eligibility is considerably narrower and is being introduced in stages.

NICE's appraisal of tirzepatide (TA1026), published in December 2024, recommends Mounjaro on the NHS only for adults with a BMI of at least 35 who also have one or more qualifying weight-related comorbidities, within a structured NHS weight-management service. The NHS rollout is phased: from June 2025, NHS prescribing opened to people with a BMI of 40 or above plus four or more qualifying conditions; from around June 2026, the threshold extended to BMI 35–39.9 with four or more conditions. Further cohorts follow through 2027. The same ethnic-background adjustment applies throughout.

What this means in practice is that many people who are eligible for Mounjaro under the licensed criteria (and through a private route) do not yet qualify for it on the NHS, or are on long waiting lists. What can make someone ineligible for Mounjaro?

A number of factors can mean a prescriber decides Mounjaro isn't suitable, even when the BMI threshold is met. Mounjaro is not recommended during pregnancy, while breastfeeding, or when trying to conceive. It is not licensed for anyone under 18. People with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia type 2 should not use it. A history of pancreatitis, certain gastrointestinal conditions, or severe kidney or liver problems will also prompt a prescriber to pause and consider carefully.

Some medicines interact with tirzepatide in ways that matter for safety. Women taking oral contraceptives should know that tirzepatide can reduce pill absorption: NHS guidance advises adding a non-oral method of contraception for the first four weeks of treatment and for four weeks after each dose increase.
