The biggest misconception: fatigue means the medicine is harming you

When tiredness hits a few days after a Mounjaro injection, many people's first instinct is that something has gone wrong. It hasn't, at least not usually. When fatigue with Mounjaro is a reason to get medical help

Most tiredness on Mounjaro is self-limiting. The following are not.

Seek urgent medical attention if fatigue is accompanied by severe, persistent stomach pain that spreads to your back, this combination can indicate acute pancreatitis, which the MHRA flagged in a Drug Safety Update for GLP-1 medicines in January 2026. Likewise, if you feel faint, cannot keep fluids down, have stopped passing urine normally, or feel extreme weakness alongside rapid heart rate, you may be becoming significantly dehydrated, which needs prompt assessment, not waiting to see if it passes. Signs of an allergic reaction (swelling of the face or throat, difficulty breathing, severe rash) require a 999 call.

Less urgently but still importantly: if you are persistently exhausted across multiple weeks at a stable dose, it is worth asking your GP to check thyroid function and iron levels. These are common and treatable causes of fatigue that have nothing to do with tirzepatide, and they are easy to miss if tiredness is automatically attributed to the injection. Prioritising sleep, eating adequate protein even when appetite is low, and avoiding alcohol (which compounds fatigue and dehydration) are all worth trying before concluding the medicine does not suit you.
