The real routes to Mounjaro in the UK, free, subsidised, and private

Does the NHS prescribe Mounjaro, and could that apply to you?

The short answer is yes, the NHS does prescribe Mounjaro, but for now, very few people are eligible. Tirzepatide was recommended by NICE in December 2024 under appraisal TA1026, and NHS England has begun rolling it out in carefully defined phases.

From June 2025, prescribing opened to adults with a BMI of 40 or above who also have four or more of five specific conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes. From around June 2026, that expanded to adults with a BMI of 35 to 39.9 who meet the same four-condition threshold. A third cohort, BMI 40-plus with three qualifying conditions, is expected from around March 2027. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance.

Even within those groups, access isn't guaranteed. GP practices can prescribe under the 2026/27 contract, but participation is optional, and wraparound diet and activity support is a condition of the prescription. Waiting times at specialist weight management services vary considerably across the country. Are there clinical trials offering Mounjaro at no cost?

Eli Lilly ran the SURMOUNT programme (a series of large phase-3 trials involving thousands of adults) before tirzepatide received its UK licence. Those trials are now closed to new participants. Post-approval studies and real-world data collection do continue, but they are not a reliable or accessible route for most people looking for treatment today.

The NIHR and research-active NHS trusts occasionally recruit for obesity pharmacotherapy studies, and some may provide the medicine as part of the trial. If you're interested in that route, the NIHR's clinical trials search and your local academic hospital are the places to look. It requires patience, strict inclusion criteria, and the willingness to be randomised, which may mean receiving a placebo. It's a legitimate option for some, but not a quick or certain one.

One practical note: trial participants are closely monitored, which is itself a form of clinical oversight. That's not incidental, it reflects the standard that any responsible prescribing of this medicine should meet, whether in a trial or through a private service like ours.

What does private Mounjaro in the UK actually cost, and what does that price include?

Since Eli Lilly raised its UK list price in September 2025, the private market has shifted noticeably. Reported prices range from roughly £149 to £375 per month depending on the dose and provider, the higher doses carry the higher cost. That's the honest market context, and it's worth knowing before you search.

What varies between providers is what that price includes. Some headline figures exclude the consultation fee, prescription charge, delivery, and any ongoing clinical support. If a website or social media account is offering Mounjaro cheaply or apparently without a clinical assessment, that is a serious warning sign. The MHRA has repeatedly warned about counterfeit weight-loss pens circulating online, fake pens have contained insulin in some cases, leading to hospitalisations. In 2025 alone, approximately 20 million doses of illegally traded weight-loss medicines (valued at around £45 million) were seized, and enforcement included the first UK raid on a site manufacturing counterfeit tirzepatide pens.

A legitimate supply always starts with a prescription following a proper clinical assessment. The pen should come from a pharmacy you can verify on the GPhC register. Discount codes for prescription medicines from unverified sellers are a red flag, not a benefit. If you encounter a suspicious seller, you can report them directly via the MHRA's Yellow Card scheme.

Storage matters too: Mounjaro pens need to be kept refrigerated at 2 to 8°C. A pen that has been stored incorrectly (or worse, is counterfeit) won't just fail to work. It could cause harm.
