The myth worth clearing up first: Mounjaro bought abroad counts towards your UK treatment

It's a reasonable assumption. You've been on tirzepatide for a few months in Hong Kong, you feel good, you know your dose, and you'd like to pick up where you left off. The misconception (and it's an understandable one) is that a prescription from one country carries weight with a UK pharmacy. It doesn't, and that's not bureaucracy for its own sake.

UK law requires that prescription-only medicines are dispensed against a valid UK prescription, written by a prescriber registered with the relevant UK body. A Hong Kong prescription isn't a valid UK prescription. More importantly, a UK Independent Prescriber reviewing you as a transfer patient isn't being obstructive; they're doing what the clinical framework asks of them: checking your current weight, your health picture, any relevant changes since your last review, and whether the dose you were on is still appropriate.

Tirzepatide is manufactured by Eli Lilly and sold as Mounjaro in the UK, the US, Hong Kong, and several other markets. The brand name is the same; the regulatory framework is not. In the UK, the MHRA licenses Mounjaro for weight management in adults with a BMI of 30 or above, or 27 and above where at least one weight-related condition is present, hypertension, high cholesterol, obstructive sleep apnoea, prediabetes or type 2 diabetes, among others. NICE's technology appraisal guidance (TA1026) then shapes how the medicine is recommended on the NHS, with stricter eligibility criteria for funded access.

Hong Kong's regulatory body, the Pharmacy and Poisons Board, may have approved tirzepatide under different indications or conditions. Dosing presentations, pen formats, and batch sourcing can all differ by market. This is one reason why medicines sourced from abroad (even genuine product) carry real risks: the supply chain, storage conditions, and lot verification that a UK-licensed pharmacy upholds simply can't be confirmed for medicines bought overseas or via informal channels.

What a UK clinical assessment looks at when you've been treated abroad

A UK prescriber reviewing someone who's been on tirzepatide elsewhere will look at broadly the same things they look at for anyone starting or continuing treatment: current BMI, weight history, any conditions that affect eligibility, current medications, and how you've responded to treatment so far. If you've been on 5mg in Hong Kong, that's clinically relevant information, it doesn't mean the UK prescriber will simply continue at that dose, but it does inform their assessment.
