Start journey Learn moreThe 5mg Mounjaro injection is the first therapeutic dose in the titration schedule, reached after four weeks on the 2.5mg starter pen. Clinical trial data published in the SURMOUNT-1 study in the New England Journal of Medicine showed that participants moving through the tirzepatide dose ladder achieved meaningful weight loss that grew with each increase, with the 5mg pen marking the point where many people notice the medicine beginning to work in earnest. Mounjaro is a prescription-only medicine; a prescriber assesses suitability before any dose is supplied.

The SURMOUNT-1 trial randomised 2,539 adults with obesity across multiple dose groups over 72 weeks. Crucially, participants didn't start on the highest dose — they climbed through the schedule in roughly four-week steps, giving the body time to adjust before each increase. By the end of the trial, average body-weight reduction at the 15mg maintenance dose reached around 20–21%, a figure that made headlines. What those headlines sometimes missed is that the lower doses, including 5mg, are the foundation on which that result is built. You cannot safely reach 15mg without the steps before it.
For most people, 5mg is where appetite suppression begins to feel tangible. The 2.5mg pen that precedes it is primarily a tolerability dose, designed to let the body adapt to tirzepatide before the medicine does its heavier work. Once you're on 5mg, both the GIP and GLP-1 pathways are being activated at a clinically meaningful level. A fuller breakdown of the Mounjaro dose schedule explains how the titration steps are structured from 2.5mg through to 15mg.
NICE's appraisal of tirzepatide, TA1026, noted that the evidence base for the dose ladder is robust, and that prescribers are expected to continue titrating patients toward the highest tolerated dose where it is clinically appropriate. The 5mg pen is a step, not a destination, though some patients remain at it for clinical reasons.
Gastrointestinal effects are the most commonly reported side effects across the Mounjaro dose range: nausea, looser stools, constipation, indigestion, burping and a general sense of stomach unsettledness. At 5mg, these are often more noticeable than they were at 2.5mg, because the medicine is now acting more fully. That said, they are typically mild to moderate and, for most people, begin to ease within one to two weeks as the body adjusts.
A question our prescribers hear regularly is whether the side effects mean the dose is wrong. Usually, no. The pattern is that they peak in the first few days after an injection, then quieten down before the next dose is due. Eating smaller portions, staying well hydrated and avoiding very fatty meals can help. If side effects are persistent or severe, that is a conversation with your prescriber, not something to manage alone.
Injection-site reactions (mild redness or tenderness where the pen was used) are also possible. Rotating injection sites between the abdomen, thigh and upper arm from one week to the next reduces this. Detailed guidance on injection technique is in the Patient Information Leaflet supplied with every pen; the Mounjaro SmPC on the eMC holds the authoritative prescribing information for clinicians.
Serious side effects are uncommon. Severe, persistent stomach pain that radiates toward the back should prompt urgent medical attention, as it can indicate pancreatitis. Reporting any suspected reaction through the MHRA Yellow Card scheme is encouraged and genuinely useful for ongoing medicine safety monitoring.
The licensed titration schedule moves from 5mg to 7.5mg after approximately four weeks, subject to tolerability. The 7.5mg dose is where many people begin to see more pronounced results. Staying at 5mg longer is sometimes the right clinical call (if side effects at 5mg are still settling, or if a prescriber judges that more time at this level is appropriate) but that decision belongs with the prescriber, not the person injecting.
Eligibility for private Mounjaro treatment requires a BMI of 30 or above, or 27 or above alongside at least one weight-related condition such as high blood pressure, type 2 diabetes, high cholesterol or obstructive sleep apnoea. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance. BMI alone never determines approval; the prescriber assesses the full clinical picture before any pen is prescribed or dispensed.
For people asking about higher doses like 10mg, the answer is the same: titration is guided by response and tolerability at each preceding step. No dose is skipped. If you're considering starting Mounjaro for the first time, the overview of tirzepatide treatment sets out the full picture, from eligibility to what to expect at each stage.
Mounjaro is a prescription-only medicine, which means it cannot lawfully be supplied without a valid prescription following a clinical assessment. The appropriate route is a consultation with a prescriber who reviews your health information, confirms eligibility, and issues a prescription if the treatment is suitable for you. A real clinician reviews every consultation, not software running a checklist.
If you order before noon on a weekday, dispatch happens the same day once your prescription is approved, and the pen arrives via DPD the following working day in plain, unbranded packaging. Plenty of people find that timing works well around ordinary life, getting an order confirmed by midday, knowing it will arrive before the next morning is over, without having arranged anything more complicated than a standard delivery.
For context on what private treatment costs at each dose, the treatment page sets out current pricing transparently. There is one price that covers everything: consultation, prescription, the pen itself, and next-working-day delivery. The FAQs address common questions about the process if you want more detail before starting. If you're ready to find out whether 5mg Mounjaro is right for you, start your free consultation and a GPhC-registered prescriber will review your answers the same day.
