A common assumption (and a question our prescribers hear most weeks) is that 15mg is simply the goal, and that any dose below it is somehow insufficient. That framing gets the logic backwards. The titration schedule exists primarily to give your body time to adjust. Gastrointestinal side effects such as nausea, indigestion and altered bowel habits are most pronounced early in treatment and after dose increases; moving more slowly reduces them. Arriving at the highest dose too fast does not improve outcomes, it tends to worsen tolerability and can lead people to stop treatment altogether.
The clinical evidence also shows meaningful weight loss at doses well below 15mg. In the SURMOUNT-1 trial, participants on 10mg tirzepatide lost an average of around 19% of body weight over 72 weeks, a result that is clinically substantial on its own. The practical point is this: your prescriber is looking for the dose at which you are losing weight at a meaningful rate and tolerating treatment well. That may be 10mg. It may be 12.5mg. It may eventually be 15mg. There is no clinical rule that says the maximum dose is always the right one for you.
Mounjaro treatment begins at 2.5mg. The opening pen's job is adjustment, settling your digestive system before the dose with any real appetite-suppressing effect takes hold. After four weeks, the dose typically steps up to 5mg, and from there the pattern continues in 2.5mg increments approximately every four weeks: 7.5mg, 10mg, 12.5mg, and finally 15mg.
That means the earliest anyone reaches the maximum dose is around five months into treatment, and many people spend longer at intermediate steps. A prescriber may slow the schedule if side effects are significant, or hold a dose if the clinical picture suggests that is the right call. Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI of 27 or above where at least one weight-related condition is present, such as high blood pressure, type 2 diabetes, high cholesterol or obstructive sleep apnoea. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance. In clinical practice, the case for moving to 15mg is strongest when someone is tolerating their current dose well, has not yet reached a stable and clinically meaningful rate of weight loss, and has no contraindications to continuing.
