What NICE and clinical trials tell us about these pens in practice

The pen is pre-filled and pre-set to the prescribed dose; there is no dial to turn, no cartridge to load. You attach a needle, select an injection site, press the button and the mechanism delivers the dose. That simplicity matters clinically: fewer steps means fewer opportunities for error. Still, correct technique (right site, right angle, rotating where you inject) does make a difference to both comfort and absorption.

A common belief is that the pen has to be used from the fridge immediately, straight from cold storage. In fact there is a limited window during which it may be held at room temperature, which is why the Patient Information Leaflet is the right place to check that detail rather than any general article. Injection sites, needle attachment and technique for Mounjaro pens

The three licensed injection sites for Mounjaro are the abdomen (avoiding a two-inch radius around the navel), the front or outer thigh, and the upper arm. Rotating between these sites (and rotating within each site) reduces the risk of local reactions and lipohypertrophy, a build-up of fatty tissue that can blunt absorption over time.

The pen itself does not come with a needle attached. You'll need to fit a compatible needle before each injection. Compatible needles for Mounjaro pens are stocked separately; a fresh needle should be used for every injection and removed immediately afterwards before capping the pen. Reusing needles blunts the tip and increases the risk of infection and injection-site pain, something our prescribers flag regularly because it seems like a small saving that isn't.

Before injecting, clean the chosen site with an alcohol swab and let the skin dry fully. Insert at a 90-degree angle (or 45 degrees if you have very little subcutaneous fat at the site), press the injection button firmly, and hold for a count of ten to ensure the full dose is delivered. You'll hear a click when the mechanism fires; keep the pen pressed against the skin until the ten seconds are up.

What to do with Mounjaro pens once the four doses are used

An empty KwikPen is a clinical sharp. UK law and NHS guidance are clear: it cannot go into household recycling or a standard bin. Used pens and detached needles belong in an approved sharps container, which most councils collect free of charge via a sharps collection scheme, contact your local authority or ask your GP surgery for details.

A 1-litre sharps container is the right size for most people on a single monthly pen; it seals securely and meets the required standard for collection. Once a container is three-quarters full, seal it and arrange collection, don't overfill it.

If you are moving to a different strength or your prescription changes, return any unused sealed pens to a pharmacy for safe disposal rather than storing them indefinitely. Pens are matched to your prescribed dose; using a different strength without clinical authorisation is not appropriate. Questions about switching or pausing treatment belong with your prescriber, not the pen's label.
