Start journey Learn moreA weight-loss plateau on Mounjaro 5mg is common, expected, and not a sign the medicine has stopped working. After an initial period of steady loss, your body adapts its energy expenditure to match reduced intake — and progress slows or stalls. This is a well-documented physiological response, not a failure of the treatment or of you. Mounjaro is a prescription-only medicine, and any changes to your dose or approach should be made with your prescribing clinician, not independently. What follows explains why plateaus happen at the 5mg stage, how to distinguish a true plateau from a normal fluctuation, and what the clinical pathway forward typically looks like.

Before drawing conclusions, it's worth distinguishing between a true plateau and the natural week-to-week noise that happens on any weight-management programme. Fluid retention after a high-sodium meal, a change in exercise load, or hormonal shifts across the month can all move the scale two to three pounds without meaning anything about the medicine. A practical check: weigh yourself on the same day each week, at the same time of day, wearing the same amount of clothing. If the number hasn't moved meaningfully in four or more consecutive weeks despite consistent effort with food and activity, that's closer to a genuine plateau. A single bad week is not.
At 5mg, many people are still in the early titration phase. Mounjaro's starting dose (2.5mg) is designed to let the body adjust rather than to drive significant weight loss. Details on that first pen and what it typically achieves are on our 2.5mg overview page. Moving to 5mg is the first real therapeutic step, and some people find they lose well at this dose before progress levels off. Others plateau sooner. Both patterns are within normal range according to the SURMOUNT-1 clinical trial programme, which randomised 2,539 adults across dose groups and observed meaningful variability between individuals at each stage.
The full Mounjaro overview covers how tirzepatide works as a dual GIP and GLP-1 receptor agonist, the dual-pathway mechanism is part of why its dose-response curve looks different from single-agonist options.
When you eat less consistently over several weeks, your body responds by reducing its total daily energy expenditure. Basal metabolic rate falls, spontaneous movement decreases, and appetite hormones shift in ways that encourage eating more. This is adaptive thermogenesis, your body defending a weight it perceives as threatened. It is not a character flaw, and it is not unique to Mounjaro. Every effective weight-loss intervention eventually runs into it.
At 5mg specifically, tirzepatide is suppressing appetite via GIP and GLP-1 receptor activation and slowing gastric emptying, but the dose may no longer be sufficient to override the body's compensatory response. The Mounjaro 5mg page has more on what this dose typically achieves and for whom the 5mg level is the right maintenance point versus a stepping stone. For some patients, 5mg is genuinely the right long-term dose. For others, the prescriber's assessment will point toward titration.
It's also worth checking in with lifestyle factors during this phase. Protein adequacy matters, tirzepatide can reduce overall intake enough that some people eat too little protein, which accelerates muscle loss and slows the metabolic rate further. Hydration, fibre, and strength-based activity all support continued progress. Neither your prescriber nor a registered dietitian should be replaced by a forum post on this, but NHS Better Health's lose-weight guidance gives a grounded starting point for the lifestyle side.
The route out of a plateau on Mounjaro 5mg depends on a clinical assessment of the whole picture, not just the scale reading. Your prescriber will typically consider how long the plateau has lasted, your current side-effect profile, your starting weight and how much you've lost, and whether lifestyle factors are contributing. From there, the most common clinical pathway is titration to 7.5mg, the next step in Mounjaro's licensed dose schedule. The 7.5mg dose page covers what that step typically involves.
NICE's appraisal of tirzepatide (TA1026) notes that if less than 5% weight loss is achieved after six months at the highest tolerated dose, continuing treatment should be reviewed. That's a consideration for much later in the titration journey, but it's useful context for understanding that the clinical system is designed to evaluate progress at every stage, not just prescribe and walk away.
What you should not do is adjust the dose yourself, skip ahead in the schedule, or source a higher dose pen independently. The titration schedule exists to manage tolerability; jumping doses increases the risk of nausea, vomiting and other gastrointestinal effects significantly. Read about how the 5mg dose is used in practice if you're unsure what the clinical rationale for your current stage is.
On the cost side, it's worth knowing that Eli Lilly raised Mounjaro's UK list price significantly from September 2025. The price increase context page explains what changed and what to expect from private providers. That's relevant if a plateau prompts you to consider changing provider at the same time as titrating dose.
If a plateau is accompanied by anything beyond the usual picture (worsening side effects, symptoms you weren't experiencing before, or concerns about the medicine itself) don't sit on it. Missing a dose or timing issues can also complicate what looks like a plateau; the late-dose guidance page addresses what to do if your injection schedule has drifted. And if you've been on 5mg for several months and genuinely feel your care isn't progressing, that's worth raising directly. Our clinical team, led by GPhC-registered Independent Prescribers, reviews every repeat prescription individually, a real prescriber reads your notes, not a queue-clearing algorithm. If you'd like to discuss titration, a stalled plateau, or your treatment plan overall, the right starting point is a fresh consultation. Speak to our prescribers and get a same-day clinical review of where your treatment stands.
