What the April 2026 GP contract change actually means

The updated QOF contract for 2026/27 gave GP practices in England the option to prescribe tirzepatide for weight management directly in primary care, without referring patients to a specialist service. That is a genuine shift: previously, NHS access to weight-loss injections sat almost entirely within secondary care and specialist tier-3 services. The change was designed to reduce pressure on those services and bring treatment closer to patients.

The operative word, though, is option. Participation is voluntary. Some practices will join; others won't, at least not immediately. Even where a GP surgery does participate, patients still need to meet the NHS criteria set out in the NICE technology appraisal for tirzepatide (TA1026), and they must enrol in structured wraparound support covering diet, physical activity and behaviour change. A willing GP and a qualifying BMI get you most of the way there, but the support requirement is non-negotiable on the NHS route. Worth checking with your own surgery directly, since NHS England has not published a public register of participating practices.

The eligibility gap: who the NHS criteria leave out, for now

NICE's current guidance (TA1026) focuses tirzepatide on adults with a BMI of at least 40 alongside four or more qualifying weight-related conditions, high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes. A second NHS cohort, covering BMI 35–39.9 with four or more of those conditions, was activated in June 2026. Lower BMI thresholds apply for South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds, reflecting the evidence that weight-related health risks arise at lower BMIs in these groups.

That still leaves a large group of people outside the current NHS criteria: someone with a BMI of 34 and one weight-related condition, for instance, or someone who meets the BMI threshold but doesn't have four qualifying comorbidities. The private licensed eligibility is broader: BMI 30 or above, or 27 or above with at least one weight-related condition such as prediabetes, hypertension, high cholesterol or obstructive sleep apnoea. A prescriber still assesses the whole picture; the number alone is never the deciding factor.

The factors that shape your decision

This is where the decision becomes genuinely personal. If your GP surgery is participating in the 2026/27 contract and you meet the NHS criteria, the NHS route makes sense: treatment is funded, wraparound support is built in, and your GP holds your full medical history. Speak to your surgery.

What to ask your GP, and what to bring to any consultation

Whether you're heading to your GP surgery or starting a private consultation, the conversation goes better with a little preparation. It helps to know your current BMI, your relevant health conditions, any medicines you take regularly, and whether you've tried other weight management approaches.
