Mounjaro requirements: what you actually need to qualify

You've searched the requirements because something has made you wonder whether you'd qualify

Maybe your GP mentioned tirzepatide but didn't take it further. Maybe you've seen the headlines, done the sums, and are trying to work out whether you'd actually be considered. That's exactly the right place to start — and the criteria are more straightforward than many people expect.

The UK licence for Mounjaro covers adults with a BMI of 30 or above, or a BMI between 27 and 29.9 where at least one weight-related condition is present. That condition list includes type 2 diabetes, prediabetes, high blood pressure, dyslipidaemia (raised cholesterol or triglycerides), obstructive sleep apnoea and osteoarthritis, among others. BMI thresholds aren't one-size-fits-all. For people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds, UK clinical guidance applies thresholds 2.5 kg/m² lower, so a BMI of 27.5 rather than 30, for instance. Meeting the BMI requirement is the start of the picture, not the whole of it. A prescriber also reviews your medical history, any medicines you currently take, and whether there are conditions that would mean Mounjaro isn't suitable for you right now. What can affect whether Mounjaro is right for you, beyond the numbers

Certain conditions don't automatically rule Mounjaro out, but they do require careful discussion. A personal or family history of medullary thyroid carcinoma, or a condition called MEN2 (multiple endocrine neoplasia type 2), means Mounjaro is contraindicated. A history of pancreatitis is another area where a prescriber will want to go carefully, the MHRA has highlighted pancreatitis as a known, if infrequent, risk with GLP-1 medicines, and that context matters when assessing someone with a prior episode.

Mounjaro is not licensed or recommended during pregnancy, while breastfeeding, or for anyone actively trying to conceive. It is not prescribed to under-18s. These are firm lines rather than areas for individual negotiation with a prescriber.

NHS criteria versus private eligibility: they're not the same thing

A point that causes genuine confusion: the requirements for Mounjaro on the NHS are considerably stricter than the licensed eligibility for private prescription. NICE's appraisal of tirzepatide (TA1026) sets out a phased NHS rollout, currently, NHS access requires a BMI of 40 or above alongside four or more specific weight-related conditions. That cohort has gradually widened since June 2025, and a second phase (BMI 35–39.9 with four or more conditions) began in June 2026.

Private prescription, by contrast, follows the licensed indication: BMI 30 or above, or 27 or above with a qualifying condition. There's no waiting list and no referral required, though clinical assessment is just as rigorous.
