A lot of people on Mounjaro assume that stomach pain coming on at night means something has gone wrong, that the injection site is infected, that the dose is too high, or that their body is rejecting the treatment. That worry is understandable, but it misreads what's happening most of the time. The overwhelming cause of overnight abdominal discomfort is the same mechanism that makes the medicine work: tirzepatide activates GIP and GLP-1 receptors that, among other things, slow how fast your stomach empties. Slow gastric emptying is the point. It stretches the feeling of fullness across more hours and reduces the urge to eat again. The trade-off is that a normal evening meal can sit in a stomach that isn't moving it through at its usual pace, which creates pressure, bloating, and a dull or cramping ache that tends to peak when you lie down flat.
Lying down removes gravity from the equation. Food and gastric acid that your posture was helping to contain are now free to press against the pyloric valve or reflux upward, which is why the discomfort so reliably appears in the hours after you go to bed. The single most effective change most people can make is eating their main meal earlier and keeping it smaller. Because gastric emptying is slower on tirzepatide, a large meal eaten at 8pm simply doesn't move on in time. Eating a lighter evening meal by 6pm or 6:30pm, and avoiding high-fat or high-fibre foods late in the day, gives the stomach a longer runway before you lie down.
