The latest Mounjaro update: what has changed in the UK

The NICE recommendation and what the trial data showed

The most consequential piece of official guidance for tirzepatide came on 23 December 2024, when NICE published technology appraisal TA1026, recommending Mounjaro for weight management in England. NICE updated that guidance in September 2025. The recommendation rests heavily on results from the SURMOUNT clinical programme, specifically SURMOUNT-1, published in the New England Journal of Medicine, which randomised 2,539 adults with obesity and found an average body-weight reduction of around 20–21% at the 15mg dose over 72 weeks.

Those figures sit at the upper end of anything previously seen in a licensed weight-loss medicine, which is why the NICE committee's decision attracted such attention. NICE recommends tirzepatide for adults with a BMI of 35 or above plus at least one weight-related comorbidity. Thresholds are 2.5 kg/m² lower for people of South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds. If a patient has not lost at least 5% of their body weight after six months at the highest tolerated dose, continuing treatment is reviewed.

The September 2025 price rise and what it means for private patients

In August 2025 Eli Lilly announced a substantial increase to Mounjaro's UK list price, taking effect 1 September 2025. The highest-dose pen, previously listed at around £122 for a four-week supply, moved to around £330. Prices for other strengths rose proportionally. This was widely reported and is now the pricing environment in which all private providers operate.

What that means in practice: anyone who researched Mounjaro costs before September 2025 will find the figures they saw are no longer accurate. Private providers typically price at or above list, and may add consultation, prescription or delivery fees on top. The NHS rollout: where things stand now

NICE's TA1026 recommendation triggered a phased NHS commissioning process, overseen by NHS England. The first cohort (adults with a BMI of 40 or above plus four or more of five qualifying conditions (high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease or type 2 diabetes)) became eligible from June 2025.

A second cohort, covering BMI 35–39.9 plus four or more of those same conditions, was activated on 23 June 2026. A further phase covering BMI 40 or above with three conditions is expected around March 2027. From 1 April 2026, GP practices can prescribe Mounjaro under the new QOF contract terms, though not every practice has opted in, availability still varies considerably by area. How the private prescription route works following these updates

These updates have driven a surge in people researching private access to Mounjaro. The legal position has not changed: tirzepatide is a prescription-only medicine, and a qualified prescriber must assess your suitability before any prescription can be written. There is no shortcut around that step, and the MHRA has warned repeatedly about fake pens and sellers offering prescription medicines without proper clinical oversight.
