Mounjaro and vulva side effects: what's reported, what's normal, and when to act

The Mounjaro Patient Information Leaflet (published on the Electronic Medicines Compendium) does not list vulval changes as a recognised side effect of tirzepatide directly. So the medicine is unlikely to be acting on vulval tissue in the way it acts on the gut or the pancreas. But that does not make your symptoms imaginary or unrelated to treatment. The more useful question is: what does weight loss do to the body, and could that explain what you're feeling?

Understanding it helps you figure out whether to watch and wait or get seen sooner.

How weight loss and hormonal shifts may affect the vulval area

Adipose tissue (body fat) is not inert storage. It produces and converts oestrogens. When fat stores reduce quickly, circulating oestrogen can dip, and the vulva and vagina are among the tissues most sensitive to oestrogen levels. Even a modest drop can reduce natural lubrication, thin the vaginal mucosa slightly, and alter local pH, making the area more prone to discomfort, irritation, or secondary infections such as bacterial vaginosis or thrush.

A second mechanism: as the body reshapes, skin folds around the groin and vulval area may shift. Most vulval discomfort linked to the indirect effects above is mild and responds to basic self-care or a short course of over-the-counter antifungal or antibacterial treatment if a secondary infection is confirmed. But some symptoms need prompt assessment.

See a GP or a sexual health clinic without delay if you notice: a blister, ulcer or open sore on or around the vulva; a lump or thickening that was not there before; bleeding that is not your period; skin colour changes (white patches, red raw areas, or darkening) that are spreading or persistent; or discharge with a colour, smell or consistency that seems unusual for you. These need examination regardless of whether Mounjaro is involved, they are not something to attribute to treatment and wait out.

Severe, persistent abdominal pain radiating to the back is a separate and more urgent concern associated with GLP-1 medicines: the MHRA issued a Drug Safety Update in January 2026 highlighting acute pancreatitis as a known but infrequent serious effect. That symptom is abdominal, not vulval, but it is worth knowing about as a Mounjaro patient, get emergency help if you experience it.

The MHRA's Yellow Card scheme exists specifically to capture side effects that may not have been identified in clinical trials.
