What the law, the regulators and the clinical evidence actually say

Why can't Mounjaro be sold without a prescription in the UK?

Tirzepatide sits in the tightest regulatory category the UK has: a Prescription-Only Medicine carrying a Black Triangle (▼) status, meaning it is under additional MHRA monitoring while post-market safety data accumulates. The Black Triangle flag matters because this is a relatively new medicine and regulators want full reporting of any unexpected effects. That monitoring only works when a prescriber is involved, someone who records what you were given, at what dose, and for what clinical reason.

The legal basis is the Human Medicines Regulations 2012. Under those regulations, supplying a POM without a prescription is a criminal offence. There is no loophole for online pharmacies, social-media sellers or grey-market importers. The MHRA is explicit that GLP-1 medicines are not assessed or approved for quick or cosmetic use, and the regulator has issued repeated public warnings about fake pens seized at UK borders and, in October 2025, raided the first illicit domestic site producing counterfeit tirzepatide. You can report suspect sellers directly via the MHRA's Yellow Card scheme.

None of this means the medicine itself is inaccessible. It means the clinical assessment step is non-negotiable, and for good reason: tirzepatide interacts with oral contraceptives during the first four weeks of treatment and after each dose increase, requires dose titration that should be supervised, and carries known risks (including pancreatitis) that a prescriber needs to screen for in advance.

What does a legitimate consultation actually involve?

A thorough online consultation for Mounjaro isn't a box-ticking exercise. A registered prescriber reviewing a Mounjaro consultation will look at your BMI, any weight-related conditions (things like hypertension, type 2 diabetes, dyslipidaemia or obstructive sleep apnoea all affect eligibility), your current medication list, relevant medical history and any contraindications listed in the SmPC. They will also check your identity. Even setting counterfeits aside, using any prescription medicine without a prescriber creates practical dangers. Tirzepatide is titrated (the dose increases in steps, typically every four weeks) and that schedule exists because the gastrointestinal side effects (nausea, vomiting, diarrhoea, reflux) are most pronounced at initiation and after increases. A prescriber can slow titration, advise on managing symptoms and spot warning signs, such as severe abdominal pain radiating to the back, that should prompt urgent medical assessment.
