Experiencing no side effects on Wegovy — is that actually possible?

The biggest myth: no side effects means Wegovy is not working

This is the misconception our prescribers hear most often. The assumption seems logical (a medicine this potent must produce some sign it is doing something) but the clinical picture is more straightforward than that.

Wegovy works by activating GLP-1 receptors in the brain and gut, slowing gastric emptying and reducing appetite signals. That mechanism does not require you to feel sick. Weight loss in the STEP 1 trial (published in the New England Journal of Medicine) averaged around 15% of body weight over 68 weeks at the 2.4mg maintenance dose. Participants who tolerated the titration well (few or no GI complaints) still achieved meaningful weight loss. The medicine was working; their bodies had simply adapted quietly.

Tolerance varies substantially from person to person. Genetics, baseline gut motility, how carefully the dose is titrated, and even what you eat during the first weeks all influence how a body responds. Having no side effects on Wegovy does not warrant asking for a faster dose increase or second-guessing whether treatment is effective. The prescriber-led titration schedule exists for good reasons, and side-effect absence is not one of the reasons to deviate from it.

What the evidence says about who gets side effects, and how often

The NHS medicines page for semaglutide lists the most common side effects as nausea, vomiting, diarrhoea, constipation, indigestion, burping, fatigue, headache, dizziness and injection-site reactions. These are categorised as common or very common, meaning they occur in more than one in ten people taking the medicine, but that also means a proportion of people do not experience them at all.

Gut symptoms tend to cluster around dose increases rather than persisting at a steady level. The titration schedule (0.25mg rising gradually to the 2.4mg maintenance dose over several months) is specifically designed to give the body time to adjust. People who eat smaller portions, avoid high-fat meals and stay well hydrated during the early weeks generally report fewer and shorter episodes. Side effects at the higher dose are not inevitable either. Some patients move through the full titration to 2.4mg (or, for those now on the newer 7.2mg single-dose pen approved by the MHRA in April 2026) without reporting significant problems. That is not unusual enough to be alarming; it is one end of a recognised spectrum.

When to get medical help, even if you have felt fine up to now

Tolerating Wegovy well most of the time is not a reason to ignore new or unusual symptoms. Several side effects require prompt attention, and knowing them in advance is simply practical.

Seek urgent medical help if you develop severe stomach pain that radiates to your back, with or without vomiting, this can be a sign of acute pancreatitis. In January 2026 the MHRA issued a Drug Safety Update specifically highlighting pancreatitis as a known but infrequent serious side effect of GLP-1 medicines. The pain is typically persistent and significantly different from ordinary nausea.

Other signs that warrant calling 111 or your GP promptly: symptoms of gallbladder problems (severe pain in the upper right abdomen, particularly after eating), signs of serious dehydration from prolonged vomiting or diarrhoea, symptoms of an allergic reaction (swelling of the face or throat, difficulty breathing, a widespread rash), or sudden changes to your vision.
