Oral Wegovy for weight loss: what the tablet actually is and how it works

Picture yourself opening the packet for the first time

You've just been prescribed oral Wegovy. The box contains small tablets, not a pen and needle, and they live in your bathroom cabinet rather than the fridge door. The routine is specific, though. You take one tablet first thing in the morning (before coffee, before breakfast, before any other medicines) with a small amount of plain water, roughly half a glass. Then you wait at least thirty minutes before eating, drinking anything other than plain water, or taking other oral medicines.

That thirty-minute window matters more than people expect. Semaglutide is not well absorbed through the gut wall on its own, so Novo Nordisk co-formulated it with an absorption enhancer called SNAC. Food, hot drinks and other tablets can all interfere with that process. The habit most people find useful: put the tablet on the kitchen counter the night before, next to a glass. When you come downstairs in the morning, swallow it straight away and set a phone timer. Simple to check, easy to build in.

The starting dose is 1.5 mg. From there, a prescriber moves you up through 4 mg and 9 mg before reaching the 25 mg maintenance dose, spending a minimum of one month at each level. What the trial evidence actually showed for the tablet

The key phase-3 study for oral Wegovy was called OASIS 4. It ran for 64 weeks and included 307 adults who were living with obesity or overweight alongside at least one weight-related condition, none of whom had type 2 diabetes. Participants followed lifestyle changes throughout.

The headline result: on average, people taking the tablet lost around 13.6% of their body weight, compared with around 2.4% in the placebo group. Among those who stayed fully adherent to the tablet throughout the trial, average weight loss reached around 16.6%. That fuller-adherence figure is worth knowing, but it applies to a specific subset, the 13.6% average is the more representative number for anyone weighing up what to expect.

The most commonly reported side effects in OASIS 4 were gastrointestinal: nausea, constipation, diarrhoea and vomiting. About 74% of participants on the tablet reported at least one GI event at some point, compared with 42% on placebo. Most were mild to moderate and tended to ease as the body adjusted, particularly after settling at a dose for a few weeks. Who the tablet is licensed for in the UK

The MHRA licence covers adults with a BMI of 30 or above, or a BMI between 27 and 29.9 where at least one weight-related condition is present, for example high blood pressure, raised cholesterol, type 2 diabetes or obstructive sleep apnoea. Treatment is meant to run alongside a reduced-calorie diet and increased physical activity rather than replace them.

A few things the tablet is not suitable for: it is not recommended during pregnancy, while breastfeeding, or for anyone trying to conceive. It is not licensed for people under 18. Certain other health conditions require a careful prescriber conversation before starting, pancreatitis history is one of them. None of this is a reason to rule yourself out before speaking to a clinician; it is a reason why clinical assessment exists.

It is also worth being clear about two related products that are sometimes confused with oral Wegovy. Rybelsus is also an oral semaglutide tablet, but it is licensed for type 2 diabetes management, not weight loss, and its dose steps (3 mg, 7 mg, 14 mg) are entirely separate from the Wegovy tablet ladder. NHS availability and the private route

At the time of the MHRA's approval, oral Wegovy was not available on the NHS. A NICE technology appraisal would normally be needed before NHS commissioning, and that process takes time. For now, access is through a private prescription following a clinical assessment.
