Pure Semaglutide and Weight Loss: Separating the Label from the Medicine

The "pure semaglutide" myth: pharmaceutical grade is already the standard

A question our prescribers hear most weeks is some version of: "Can I get pure semaglutide rather than the branded version?" It is a reasonable thing to wonder, but it rests on a misconception. Semaglutide is a synthetic GLP-1 receptor agonist with a fixed molecular structure, it does not come in "pharmaceutical grade" and a purer upgrade. Every milligram of semaglutide in a licensed Wegovy pen or Wegovy tablet is manufactured to the same exacting standard, validated by the MHRA before any product reaches a UK pharmacy shelf.

What people are often actually looking for when they search for semaglutide pure or pure semaglutide is either a cheaper source or a way around a prescription. Those two things are connected, and both carry real risk. Unlicensed semaglutide products (sold by unverified online sellers as raw compound or as unmarked injections) have not been tested for human use in their supplied form. Concentration can be wildly inaccurate. In 2023 and through 2025 the MHRA seized around 20 million doses of illegally traded weight-loss medicines with an estimated street value of around £45 million, including counterfeit pens containing insulin rather than semaglutide. The only safe route to semaglutide for weight loss is through a legitimate prescription, dispensed by a pharmacy you can verify on the GPhC register. The label matters precisely because it tells you the product has passed scrutiny that an unlabelled vial has not.

What licensed semaglutide products exist in the UK right now

Two forms of semaglutide are licensed in the UK specifically for weight management. The first is Wegovy injection, a once-weekly subcutaneous pen available in doses from 0.25 mg up to 2.4 mg, with a higher 7.2 mg dose approved by the MHRA in January 2026 and a dedicated single-dose 7.2 mg pen approved on 14 April 2026. In the STEP 1 trial, published in the New England Journal of Medicine, participants using 2.4 mg weekly lost around 15% of body weight on average over 68 weeks alongside lifestyle changes.

The second is Wegovy tablets, which the MHRA approved on 11 June 2026, making the UK the first country in Europe to license an oral GLP-1 medicine for weight loss. The tablet uses the same semaglutide molecule co-formulated with an absorption enhancer. In the OASIS 4 phase-3 trial, participants lost around 13.6% of body weight on average over 64 weeks; among those who adhered fully to treatment the figure was approximately 17%. Neither product is available on the NHS without referral through specialist weight management services under NICE guidance (TA875). Why the framing of "purity" matters for your safety

Searching for pure semaglutide often reflects a legitimate frustration, branded medicines are expensive, waiting lists for NHS treatment are long, and the internet is full of sellers promising the same ingredient at a fraction of the cost. That frustration is understandable. But pharmaceutical purity is not a bonus feature you are missing out on by buying licensed Wegovy; it is a baseline that licensed products meet and unlicensed ones do not.

The MHRA's Yellow Card scheme allows anyone to report suspected fake or substandard medicines, and the agency actively pursues sellers of unlicensed injectable compounds. Receiving an injection of the wrong concentration (whether too weak to be effective or too strong to be safe) carries the same risk regardless of what the seller calls the compound. Contamination with bacteria or endotoxins is a documented risk with unvalidated preparations.

What the licensed product schedule looks like in practice

For people who do proceed with licensed semaglutide, the dosing schedule is a titration set by the prescriber, starting low to let the body adjust before stepping up. The most common side effects are gastrointestinal: nausea, loose stools and reduced appetite are typical at the start of treatment or after a dose increase. These usually settle within a week or two.
