A step-by-step guide to attaching, using and disposing of screw-on needles for the Mounjaro KwikPen

Step 1: Gather everything before you start

Before you take the pen out of the fridge, collect what you need: a compatible screw-on pen needle, an alcohol swab, and an approved sharps container within arm's reach. Alcohol swabs are used to clean the injection site, not the pen needle itself. Check the pen's label (name, dose, expiry) and let it sit at room temperature for about 30 minutes if it has just come out of the fridge. Cold medicine injected straight from the fridge is one of the more common causes of injection-site discomfort, and it costs nothing to wait. Keep your sharps container somewhere stable nearby. The moment the needle comes off the pen, it needs to go straight in. You won't regret having it open and ready rather than fumbling with the lid one-handed.

Step 2: Attaching the screw-on needle correctly

Peel back the outer foil on a fresh needle, do not remove the caps yet. Hold the pen upright and align the needle straight with the pen's needle thread at the tip. Screw it on clockwise, firmly but without overtightening; it should feel snug after three or four turns. Do not force it. An off-angle needle can leak medicine or cause the pen to dose inaccurately.

Once attached, remove the outer cap first (keep it for removal later), then the inner cap. The needle is now exposed and ready. If you're choosing between lengths, most adults using Mounjaro find shorter needles easier and less uncomfortable. 6mm needles are widely used, though 5mm options suit many people just as well. Step 3: Removing the needle safely after each injection

Immediately after injecting, replace the outer needle cap (the one you kept from step 2) over the exposed needle. Use the cap, never your fingers. Unscrew the capped needle from the pen and drop it directly into your sharps container. Do not resheath by holding the inner cap; do not try to snap the needle off. That's the whole process. It takes about ten seconds once you've done it a couple of times.

Reusing a needle is a common mistake worth flagging directly: a used needle is no longer sterile, the tip is microscopically blunted after a single injection, and reuse increases the risk of injection-site infection and lipohypertrophy, localised thickening of fatty tissue that affects medicine absorption. Step 4: Disposing of sharps and staying legal

Used pen needles are clinical sharps waste. They cannot go into a wheelie bin, a carrier bag, or recycling. In the UK, your local council or GP surgery can usually advise on sharps disposal services; many councils offer free sharps bin collection for people on prescribed injectable medicines. A rigid, puncture-resistant sharps container is the correct receptacle, and once it reaches the fill line (never above it) it should be sealed and disposed of through your council or pharmacy take-back scheme.
