HFpEF accounts for roughly half of all heart failure cases in the UK, yet until recently no medicine had convincingly shown it could reduce symptoms at scale. The heart pumps normally but the muscle stiffens and fills poorly, and obesity makes it worse by increasing filling pressures and inflammation. That is the precise population STEP-HF recruited.
Over 52 weeks, participants taking semaglutide 2.4mg once weekly scored around 7 points better on the Kansas City Cardiomyopathy Questionnaire (a validated, patient-reported symptom and quality-of-life tool) compared with those on placebo. A 5-point change is considered clinically meaningful. Six-minute walk distance also improved significantly. These are hard endpoints, not just averages drifting in the same direction. The study did not find a statistically significant reduction in hospitalisations for heart failure or deaths, so claims that semaglutide 'prevents cardiac events' in HFpEF go beyond what STEP-HF showed. That nuance matters when interpreting headlines.

Losing around 13% of body weight in a year would itself be expected to ease HFpEF. Reduced adipose tissue lowers circulating inflammatory markers, reduces ventricular filling pressure and lightens the mechanical load on a heart that is already struggling to relax. Researchers acknowledged this confounding in their analysis.
What makes STEP-HF interesting is that symptom improvement appeared to exceed what weight loss alone would predict, which has led some cardiologists to speculate about direct anti-inflammatory or natriuretic effects of GLP-1 receptor agonism. Those mechanisms are still being studied; they are not established. The SELECT trial (a separate, much larger cardiovascular outcomes study in over 17,500 adults with pre-existing cardiovascular disease) did find a significant reduction in major adverse cardiovascular events with semaglutide 2.4mg, lending broader context to cardiac-related interest in this drug. For the specific question of semaglutide in heart failure with reduced ejection fraction, the picture is more complicated and the evidence thinner, that is a different condition with a different pathophysiology.
None of this changes the regulatory status. Semaglutide is licensed in the UK as Wegovy for weight management in eligible adults, not as a cardiology medicine. NICE's recommendation under TA875 reflects this: semaglutide for weight management, within specialist services, for up to two years, with a comorbidity profile and BMI threshold that a prescriber assesses.
