Activating both pathways together appears to produce greater average weight loss in head-to-head trials, the SURMOUNT-5 study found tirzepatide outperformed semaglutide 2.4mg on mean weight reduction over 72 weeks.

What does the mechanism mean for side effects and practical timing?

Because semaglutide slows gastric emptying, the most common early side effects are gastrointestinal: nausea, loose stools, constipation, or some combination, particularly after starting or after a dose increase.
