What changes when a semaglutide injection enters muscle

Muscle tissue has a richer blood supply than subcutaneous fat, so absorption into the bloodstream tends to be faster. For a medicine calibrated to a weekly subcutaneous release curve, a sudden faster uptake is not the goal, and it is not the profile that has been studied for safety and efficacy. More practically, injecting into muscle is simply more painful. Skeletal muscle contains more nerve fibres sensitive to mechanical and chemical stimulation, which is why intramuscular injections are typically performed with longer needles and specific clinical technique. A Wegovy pen's needle is designed for subcutaneous depth; pressing firmly into a lean site (particularly the upper arm or a thin thigh) raises the odds of inadvertently reaching muscle.

Local reactions including bruising, haematoma and prolonged soreness are more common when the needle enters muscle. There is also a small risk of injecting into a blood vessel if the technique is off. None of this means a single misplaced injection is a medical emergency, but it does mean technique is worth getting right from the start. How to be sure the needle lands in the right layer

The practical answer is a combination of site selection, skin preparation and the pinch technique. On the abdomen, aim for the area around the navel but at least two inches away from it; pinch a fold of skin and fat between thumb and forefinger before inserting the pen. Signs that warrant contacting a clinician sooner include a lump or swelling that grows rather than shrinks, skin that becomes hot and red beyond a small area, or pain that worsens after 48 hours rather than improving.
