Semaglutide oral dosage for weight loss: how the Wegovy tablet schedule works

Why the dosage ladder exists, and what each step is actually doing

A question our prescribers hear most weeks is some version of: "Can I just start on the higher dose?" The short answer is no, and understanding why makes the whole schedule easier to stick with.

Oral semaglutide works by activating GLP-1 receptors involved in appetite regulation and gastric emptying. That mechanism is effective, but it also produces the nausea, loose stools and indigestion that most people notice when they first start or step up. The dose chart is not arbitrary, each minimum-four-week period at 1.5mg, 4mg and 9mg gives your gut time to adapt before the next increase.

The 25mg maintenance dose is the one the MHRA licensed for long-term weight management. Clinical trial data (the OASIS 4 phase-3 study, reported at approval by Novo Nordisk) showed an average body-weight reduction of around 13.6% over 64 weeks compared with around 2.4% on placebo, taken alongside lifestyle changes. Among participants who stayed fully adherent throughout the trial, average loss reached approximately 16.6%. Both figures come with the same caveat: individual results vary, and the medicine works best when diet and activity changes run alongside it.

People already established on the 2.4mg weekly Wegovy injection may be able to move directly to the 25mg tablet, but that transition is a clinical decision, not a self-managed switch. The practical side the dose card does not tell you

Getting the timing right matters more with the Wegovy tablet than with most once-daily medicines. The tablet needs to be taken first thing in the morning, on a completely empty stomach, with a small amount of plain water, no more than about 120ml. You then wait a minimum of 30 minutes before any food, any other drink (including coffee), or any other oral medicine. Miss that window and absorption drops significantly.

The good news is that storage is straightforward. Unlike the injectable semaglutide options, the tablet does not need refrigeration. Room temperature is fine, which makes it considerably easier to manage on trips or at work.

Swallow the tablet whole. Do not crush, chew or split it, the absorption mechanism depends on the tablet's intact coating. Who the oral dosage schedule applies to, and who it does not

The Wegovy tablet licence covers adults with a BMI of 30 or above, or 27–29.9 alongside at least one weight-related condition such as high blood pressure, raised cholesterol or obstructive sleep apnoea. Lower BMI thresholds may apply for some ethnic backgrounds under UK guidance, your prescriber will apply the correct threshold during assessment.

It is not licensed for people under 18, during pregnancy, while breastfeeding, or for those trying to conceive. If any of those situations apply to you now or might do soon, that is a conversation to have with your prescriber before starting.

A separate point worth making clearly: Rybelsus is also oral semaglutide, but it is licensed for type 2 diabetes at doses of 3mg, 7mg and 14mg. Those strengths are not steps on the Wegovy weight-loss ladder, and Rybelsus is not approved or appropriate for weight management. If you have seen references to 3mg or 7mg oral semaglutide in a weight-loss context, that content is drawing on the wrong licence. Getting a prescription for the Wegovy tablet through a regulated pharmacy

The Wegovy tablet is a prescription-only medicine. In the UK, the only legal route to it is a prescription following a clinical assessment by a qualified prescriber, a pharmacist, doctor or nurse independent prescriber registered with the appropriate regulatory body.
