The case for semaglutide rests on solid evidence. In STEP 1 (a 68-week randomised trial) adults taking 2.4 mg semaglutide weekly lost an average of around 15% of their body weight, compared with about 2.4% in the placebo group. The full STEP 1 paper is published in the New England Journal of Medicine and underpins NICE's position.
NICE reviewed that evidence and, in Technology Appraisal TA875, recommended semaglutide for NHS use in adults with a BMI of 35 or above who also have at least one weight-related condition, things like high blood pressure, type 2 diabetes or obstructive sleep apnoea. For people from South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, those BMI thresholds are 2.5 kg/m² lower. NHS access, however, requires a specialist weight management service referral, and waiting lists remain long in many areas.
Private prescribing follows the licensed criteria in the Wegovy SmPC: adults with a BMI of 30 or above, or 27 and above with at least one weight-related condition. Semaglutide NHS prescription: what the rollout actually means right now
Getting a semaglutide NHS prescription for weight loss is, for most people, not straightforward at the moment.
