The BMI figure is the starting point for a clinical conversation, not a password. A prescriber weighing up whether semaglutide is right for you will look at your full health picture: current medicines, any history of pancreatitis or gallbladder disease, thyroid conditions, kidney function, and whether you are pregnant or planning to become so.

Semaglutide is a GLP-1 receptor agonist, which means it slows how quickly your stomach empties and acts on appetite signals in the brain. One more thing worth knowing: Ozempic is also semaglutide, but it holds a licence for type 2 diabetes management, not weight loss. The two are not interchangeable for this purpose, regardless of what you may have read.

BMI thresholds: what the licence actually says

The licensed eligibility for Wegovy in the UK covers adults with a BMI of 30 or above, or 27 to 29.9 where at least one weight-related condition is present. Qualifying conditions include type 2 diabetes, hypertension, dyslipidaemia, obstructive sleep apnoea, and cardiovascular disease, among others. So if your BMI sits at 28 and you have been told your blood pressure is raised, that combination may meet the threshold. Worth checking properly rather than ruling yourself out.

The ethnic-background adjustment is significant and often overlooked. UK clinical guidance recognises that health risks associated with excess weight can occur at lower BMI values in some populations, so the thresholds sit roughly 2.5 kg/m² lower for people from certain backgrounds. BMI alone never guarantees approval. Two people with an identical BMI can have very different health profiles, and the prescriber's job is to assess both. That is not a hurdle to clear, it is the thing that keeps treatment safe.

Who cannot take semaglutide

There are clear groups for whom semaglutide is not appropriate, and it is worth being straightforward about them. The medicine is not licensed for under-18s. It is not recommended during pregnancy, while breastfeeding, or for people actively trying to conceive, the MHRA advises using effective contraception during treatment and for a period after stopping before attempting to get pregnant. A history of pancreatitis is another area where the prescriber will want to look closely. Severe gastrointestinal conditions that might be made worse by slowed gastric emptying are also relevant to the assessment.
