Should Mounjaro be kept in the fridge? What the storage rules actually mean

The core decision: does my pen need to go back in the fridge after use?

This is the question our prescribers hear most weeks, and it deserves a straight answer. A Mounjaro KwikPen that has not yet been used is a refrigerated medicine, store it between 2°C and 8°C, ideally in the body of the fridge rather than in the door, where temperatures fluctuate with every opening. Keep it away from the freezer compartment entirely; freezing inactivates the medicine and there is no safe way to revive a frozen pen.

Once you have taken your first dose from a pen, the same temperature rules apply for the doses that remain. You do not need to return the pen to the fridge between weekly injections if you are within the room-temperature window, but you do need to know what that window is. Read it when your first pen arrives rather than searching online in a hurry the day your pen has been sitting on the windowsill. If a pen has been left out longer than permitted, do not inject it. Call your prescriber or pharmacist, they would rather field that call than have you inject a degraded medicine. What happens when Mounjaro gets too warm or too cold?

Tirzepatide, the active molecule in Mounjaro, is a large peptide. Like most peptides it is sensitive to temperature extremes in both directions. Freezing can cause the solution to crystallise and the protein structure to unfold irreversibly, the pen may look normal but the medicine is no longer intact. Sustained heat (above 30°C for extended periods, for instance) accelerates degradation in a similar, if slower, way.

Visually, a damaged pen may look identical to an intact one. There is no colour change or cloudiness that reliably signals the problem, which is why the time-and-temperature rules in the leaflet exist: they are the safety net, not a rough guide. Practical tip: the back wall of a fridge often runs slightly below 2°C, especially in older models. A fridge thermometer costs a few pounds and removes any guesswork. Travelling with Mounjaro: planes, holidays and days away from home

This is where storage decisions become genuinely complicated. If you are flying, Mounjaro must travel in your hand luggage, not the hold, where cargo temperatures routinely fall well below freezing. Declare it at security if asked, it is a prescription medicine and TSA-style rules in the UK mean liquids over 100ml are permitted in hand luggage when they are medicines, provided you carry evidence of the prescription.

On longer trips, a small insulated cool bag with a gel ice pack (not in direct contact with the pen) keeps things within range on journeys of several hours. Pharmacies, hotels and GP surgeries abroad sometimes provide temporary refrigeration, ask before you need it. In hot climates, leaving a pen in a parked car for an afternoon is enough to compromise it; the car interior can reach 50°C or above in summer sun.

For a single day out (a long walk, a family day at the coast) the room-temperature window in the leaflet is your guide. Disposal, accessories and keeping storage practical

Used needles must be placed in an approved sharps container immediately after each injection, not into household waste, not into the original pen box. A one-litre sharps container sits neatly in most bathroom cabinets and removes any ambiguity about how to dispose of needles safely. Councils in England accept filled containers at household waste sites.

The pen itself should be stored with a new needle attached only at the point of use; leaving a needle on between doses introduces a risk of contamination and air entering the pen. Wipe the injection site with an alcohol swab and allow the skin to dry before injecting, alcohol residue can sting if the needle goes in while it is still wet.
