Sickness and Diarrhoea on Wegovy: Separating the Expected from the Serious

The myth: sickness on Wegovy means the medicine isn't right for you

Plenty of people feel queasy in the first week or two and assume their body is rejecting the treatment. It isn't. Semaglutide slows the rate at which your stomach empties and acts on hunger-regulating receptors in the gut and brain. Both mechanisms can cause nausea, loose stools or vomiting, not because something has gone wrong, but precisely because the medicine is doing what it is designed to do.

In the STEP 1 trial, published in the New England Journal of Medicine, the majority of participants on semaglutide 2.4mg experienced at least one gastrointestinal side effect at some point during the 68-week study. Most were mild to moderate and time-limited. Discontinuation because of gut symptoms was far less common than the number who experienced symptoms in the first place, meaning most people moved through the difficult early patch and continued treatment.

The dose-escalation schedule exists partly for this reason. Starting at 0.25mg and increasing roughly every four weeks gives your body a graduated introduction rather than a sudden high exposure. Side effects tend to cluster around each new dose level, then ease. So the discomfort is real and worth taking seriously. It is not, however, an automatic reason to stop.

What you can actually do when nausea and diarrhoea hit

There is no single trick that works for everyone, but the things most consistently reported as helpful are practical and low-risk. Eating smaller portions more frequently rather than large meals puts less demand on a stomach that is already emptying slowly. Foods that are bland, low in fat and lower in fibre tend to be easier to tolerate in the first weeks. Fizzy drinks and alcohol are worth avoiding when nausea is bad, both can worsen symptoms. Staying upright for an hour or two after eating matters more than people expect.

Loose stools cause fluid loss quickly, and semaglutide-related diarrhoea can be more frequent than people anticipate. Plain water, diluted juice and oral rehydration sachets all help; caffeine and very sweet drinks can make things worse. If you are managing to keep fluids down and the diarrhoea is not severe, home management is usually appropriate while things settle.

Timing your injection for a day when you have fewer commitments (a Friday evening, for instance) can mean the worst of any post-dose nausea passes over a couple of days when the pressure is lower. When sickness and diarrhoea on Wegovy need medical attention

This is the part that matters most. Most gut symptoms on Wegovy are self-limiting; a few are signals that need prompt attention.

Get medical help the same day if you have severe, persistent stomach pain, particularly if it radiates toward your back and comes with vomiting. In January 2026, the MHRA issued a Drug Safety Update highlighting acute pancreatitis as a known, infrequent but potentially serious side effect of GLP-1 medicines including semaglutide. The combination of severe abdominal pain and vomiting is the pattern to act on urgently.

Seek medical advice also if vomiting or diarrhoea is severe enough that you cannot keep any fluids down for more than a day, if you feel signs of dehydration (dizziness on standing, very dark urine, confusion), or if the symptoms have not improved at all after two weeks at a stable dose. Dehydration in this context is not just uncomfortable, it can put strain on the kidneys, which is why the NHS semaglutide guidance specifically flags it.

Signs of a serious allergic reaction (swelling of the face, throat or tongue, difficulty breathing, rapid heart rate) are a 999 situation.
