Taking Weight Loss Injections: What to Expect From Your First Dose Onwards

Once-weekly injections, same day each week: both Mounjaro (tirzepatide) and Wegovy (semaglutide) are injected subcutaneously (under the skin) once a week, rotating between the abdomen, thigh and upper arm.
Treatment starts at a low tolerability dose: the first pen's job is to let your body adjust, not to produce maximum results immediately; dose increases are decided by your prescriber.
GI effects are common early on: nausea, loose stools or constipation are the most reported side effects in the first few weeks; for most people they ease as the body adapts.
Pens must be kept refrigerated: store between 2–8°C until you are ready to use; specific out-of-fridge time limits are set out in the Patient Information Leaflet for your medicine.

You have your first pen in the fridge, an injection day circled on your calendar, and a handful of questions nobody quite answered at the pharmacy. Taking weight loss injections like Mounjaro or Wegovy is straightforward in practice, but knowing what to expect before you start makes the difference between a settled first week and an anxious one. Both medicines are prescription-only, assessed and issued by a registered prescriber after a clinical review of your health — they are not something to begin without that oversight. The guide below walks through the practical reality, from the first injection to building a routine that fits your life.

How to inject, build a routine, and handle what comes up along the way

The first injection: what it looks and feels like in practice

Picture this: it is Sunday morning, the pen has been sitting in the fridge door since it arrived. You take it out about thirty minutes before you plan to use it, because a cold pen is more uncomfortable than one that has reached room temperature. You choose your injection site — the abdomen, just to the side of the navel, works well for most people starting out. You clean the skin with an alcohol swab, let it dry for a few seconds, pinch the skin gently, press the pen firmly against it and press the button.

That is largely it. The needles on pre-filled GLP-1 pens are short and fine; most people describe the sensation as a mild pressure rather than a sharp sting. Each Mounjaro KwikPen delivers four weekly doses before it is empty. Wegovy comes as individual pre-filled pens. The full injection technique, including how long to hold the pen in place, is set out in the NHS tirzepatide patient page and in the leaflet inside the box, read it before your first injection, not after.

After injecting, dispose of the needle safely. A sharps container is the correct option; household bins are not suitable for used needles.

Building a weekly routine that actually holds

Picking the same day every week is the single most practical thing you can do. It removes the decision entirely. A lot of people anchor it to a weekend morning, lower stress, less time pressure, and you are more likely to notice any nausea before a demanding day at work.

Rotating injection sites matters more than it sounds. Using the same spot repeatedly can lead to skin changes that affect how well the medicine is absorbed. Keep a loose mental note: abdomen this week, thigh next, upper arm the one after. You do not need a spreadsheet; just move around.

If you are travelling, taking weight loss injections on holiday is manageable with a little planning, the main requirement is reliable cold storage for your pens. For longer trips, a travel cooler designed for medicines works well. The Patient Information Leaflet for your specific medicine states exactly how long the pen can be kept outside refrigeration; stick to that window rather than estimating.

The foods that sit well alongside treatment are worth thinking about too. GLP-1 medicines slow gastric emptying, which means large or very fatty meals can feel heavy in a way they did not before. Smaller portions, more protein, and not skipping hydration are the practical adjustments most people land on.

Side effects in the first weeks: what is normal and what is not

Nausea is the most commonly reported effect, particularly in the days after a new or increased dose. It is usually mild to moderate and tends to lessen within one to two weeks as the body adjusts. Loose stools, constipation, indigestion and fatigue also appear on the common-effects list; the full side-effect picture has more detail if you want to go deeper.

Most of these settle. Staying hydrated helps, especially if you are experiencing any GI symptoms. Eating slowly and keeping meals modest around injection day is a small but useful habit.

Some symptoms need prompt medical attention. Severe, persistent stomach pain that reaches the back (with or without vomiting) should not be waited out; it can be a sign of pancreatitis, which the MHRA highlighted in a Drug Safety Update for GLP-1 medicines (January 2026) as a known, infrequent but serious side effect. Signs of a significant allergic reaction, gallbladder pain or symptoms of dehydration from prolonged vomiting or diarrhoea also warrant medical help the same day. If you are ever unsure, your prescriber or a pharmacist can help you decide.

Yellow Card, run by the MHRA, lets patients report suspected side effects directly: yellowcard.mhra.gov.uk. It is a straightforward online form and the information genuinely feeds into post-market safety monitoring.

What the treatment does not do automatically, and where clinical support fits in

These medicines work best alongside meaningful changes to how you eat and move. That is not a caveat buried in small print; it is how the clinical trials were designed and how NICE's recommendation of tirzepatide (TA1026) frames the treatment. The injections reduce appetite and slow how quickly food leaves the stomach, which makes the behavioural changes easier, but the changes still need to happen.

Dose increases are a prescriber decision, not a patient one. There is a licensed schedule for both Mounjaro and Wegovy, starting at a low dose for tolerability and moving upward in steps. The timing and whether a particular increase is appropriate for you depends on how you are responding, clinically and in terms of tolerance. Never adjust the dose outside what has been prescribed.

Questions about missed doses, what happens if you inject at the wrong site by mistake, or whether a particular symptom is expected: these belong with your prescriber, not a forum. At nume, our prescribers are available seven days a week through aftercare, a question on a Sunday evening does not have to wait until Monday. If you are curious about what how long treatment typically lasts looks like, or how the process of getting started works, the safety evidence behind these medicines is worth a read before your consultation.

When you are ready to explore whether Mounjaro or Wegovy is clinically suitable for you, our weight loss injections page covers how both options work before you start your free consultation with our prescribers, reviewed the same day, no waiting list.
