The SURMOUNT-1 trial, published in the New England Journal of Medicine, enrolled 2,539 adults and followed them for 72 weeks across all tirzepatide dose levels. Fatigue appeared in the adverse-event data at every dose, most prominently in the weeks immediately after starting treatment or after moving to a higher strength. This pattern closely mirrors what is seen with other GLP-1 medicines and is consistent with the pharmacology: tirzepatide acts on two gut-hormone receptors (GIP and GLP-1), slowing how quickly the stomach empties, dampening appetite and affecting how the body uses energy. Each of those changes can leave someone feeling flat or low on energy, particularly before the body settles into the new baseline.
The NHS medicines page for tirzepatide lists tiredness alongside nausea, diarrhoea and dizziness as common effects. Common in this context means reported by more than one in a hundred people — not rare, and not unexpected. That framing matters: if you feel tired on Mounjaro, it does not usually signal that something has gone wrong. It signals that your body is responding to a significant physiological shift.

There is rarely a single cause. In practice, tiredness on tirzepatide tends to come from several overlapping factors that are worth separating out, because the remedies differ.
Calorie reduction: Mounjaro substantially reduces appetite. That is the point. But a steep drop in food intake, particularly when it happens quickly, means less fuel reaching your muscles and brain. People who are eating very little (sometimes unintentionally) often report the heaviest fatigue. Keeping protein intake adequate (eggs, fish, legumes, Greek yoghurt) and eating regular small meals rather than skipping them makes a measurable difference for many.
Dehydration: Nausea and reduced appetite can suppress thirst signals. If you are also experiencing any gastrointestinal upset, fluid losses compound the problem. Mild dehydration alone causes fatigue, reduced concentration and low mood. Sipping water steadily through the day, rather than drinking large amounts infrequently, tends to help.
Blood sugar changes: Even without diabetes, tirzepatide affects glucose metabolism. A more stable, lower blood-sugar environment is the goal of treatment, but some people feel sluggish during the adjustment. This is generally temporary.

Most fatigue on Mounjaro is mild to moderate and improves within days to two weeks. There are situations, though, where you should contact a healthcare professional without delay rather than waiting to see if it resolves.
Severe and persistent exhaustion combined with significant vomiting or diarrhoea can be a sign of dehydration that needs clinical assessment. Tiredness alongside severe stomach pain (especially pain that radiates toward the back) warrants urgent medical attention: the MHRA issued a Drug Safety Update in January 2026 specifically highlighting acute pancreatitis as a known, though infrequent, risk with GLP-1 medicines. That combination of symptoms should never be attributed to ordinary fatigue and left to settle.
Sudden, unusual breathlessness alongside tiredness, or fatigue so pronounced it stops you functioning day to day after the adjustment window has passed, also warrant a conversation with your prescriber. Gentle activity, even short walks, often counteracts the low-energy feeling better than rest alone. Heavy or intense exercise during the first days after a dose increase is worth avoiding if fatigue is significant.
