Tirzepatide peptide 5mg: what changes at this dose and what to expect

5mg is the first dose in the Mounjaro schedule that is considered therapeutic rather than a tolerability starter.
Titration is managed by your prescriber, not by self-adjustment — do not change your dose without clinical input.
The most commonly reported side effects at 5mg are gastrointestinal: nausea, looser stools, reduced appetite and occasional burping, usually mild and short-lived.
Clinical trial evidence for tirzepatide at higher doses is substantial; results across the full dosing range are published in the SURMOUNT programme trials in the New England Journal of Medicine.

The 5mg dose of tirzepatide peptide is the first therapeutic step in the Mounjaro schedule — the point where treatment moves beyond the starter phase and begins to do clinical work. After four weeks at 2.5mg (a tolerability period, not a treatment dose), a prescriber typically moves patients to 5mg for the next four weeks, and this is where most people start noticing a meaningful change in appetite. Tirzepatide is a prescription-only medicine; a GPhC-registered prescriber decides whether 5mg is right for you and when to move further.

How the 5mg step fits into the full tirzepatide course, and why it matters

Why does the schedule move through doses rather than starting higher?

Tirzepatide works on two gut-hormone pathways (GIP and GLP-1) that together reduce appetite, slow how quickly food leaves the stomach, and influence blood-sugar regulation. That dual action is effective, but it also means the body needs time to adjust. Starting low and stepping up is a deliberate design choice, not a delay tactic.

The 2.5mg first pen is there purely to let your digestive system settle. Moving to 5mg is the prescriber's signal that tolerability has been established and clinical work can begin. Most people on the Mounjaro treatment schedule notice the appetite change becoming more noticeable at this point, eating smaller portions without effort, feeling full earlier, and thinking about food less between meals.

The full schedule continues through 7.5mg, 10mg, 12.5mg and 15mg, each offered after a minimum of four weeks at the previous level. You can read about how prescribers approach that progression on our tirzepatide dosing guide. Not everyone needs to reach the top; some patients respond well and stay at 5mg or 7.5mg long-term. That decision sits with the prescriber reviewing your progress.

The NHS patient information for tirzepatide, published at nhs.uk/medicines/tirzepatide, sets out the licensed dosing schedule in plain language and is worth reading alongside any clinical conversation.

What does the evidence say about 5mg specifically, and how does it compare to higher doses?

The SURMOUNT-1 trial, published in the New England Journal of Medicine, tested tirzepatide at 5mg, 10mg and 15mg against placebo across 72 weeks, in thousands of adults with obesity and no diabetes. At 5mg, participants lost an average of around 15% of their body weight, a clinically significant result in its own right. Those at 15mg averaged around 20–21% loss over the same period.

That gap matters in context. For someone at a lower BMI threshold, 15% may be exactly what they need. For others, the prescriber may recommend stepping further once 5mg has been tolerated for a full four weeks. The point is that 5mg is not a consolation dose, it is a proven therapeutic level that simply sits below the ceiling.

If you want to understand how the higher end of the range performs, the 15mg tirzepatide page covers the trial evidence at that dose in detail. For a sense of what the 10mg step looks like in practice, our 10mg tirzepatide page picks that up.

Cost is part of the picture too. Since Eli Lilly revised UK list prices in September 2025, private treatment costs have shifted at every dose. The Mounjaro price increase explainer covers what changed and what it means for private patients.

What side effects should people expect when they move to 5mg?

The most common effects at 5mg (nausea, loose stools, reduced appetite, indigestion, fatigue) are the same GI profile seen across the tirzepatide schedule, and they are most noticeable in the days after a dose step-up. For most people, they settle within a week or two as the body adjusts.

Keeping meals smaller, eating slowly and avoiding rich or fatty foods during the adjustment window helps considerably. Staying well hydrated matters too, particularly if nausea reduces the urge to drink. Your Patient Information Leaflet, which comes with every pen, gives detailed practical guidance that is specific to you in a way that a content page cannot be.

A small number of people experience side effects that do not settle. Severe, persistent stomach pain that extends to the back should prompt urgent medical attention, this symptom, which can indicate pancreatitis, is one the MHRA specifically highlighted in its January 2026 Drug Safety Update on GLP-1 medicines. Gallbladder discomfort and signs of a serious allergic reaction (swelling, breathing difficulty, rash) also warrant prompt medical contact. Report any suspected side effect via the MHRA Yellow Card scheme.

If you are concerned about a side effect or want to discuss whether to hold at 5mg rather than step up, our aftercare team is available seven days a week.

Who actually gets prescribed 5mg, and for how long?

Under its UK licence, tirzepatide is available to adults with a BMI of 30 or above, or a BMI of 27 or above with at least one weight-related condition such as high blood pressure, type 2 diabetes, dyslipidaemia or obstructive sleep apnoea. Lower BMI thresholds apply for some ethnic backgrounds under UK clinical guidance. A BMI figure alone, though, has never been the whole story, your prescriber weighs up your full medical picture.

On the private route through a service like nume..., through a service like ours at nume, there are no NHS waiting lists or phased eligibility cohorts to navigate. A prescriber reads your consultation the same day, and if 5mg is the right starting point clinically, that is what gets prescribed.

Some patients stay at 5mg for longer than four weeks if tolerability needs more time; others move through it quickly. Dose timing is not always neat. More than one patient has asked whether it matters if their step-up coincides with a holiday or lands mid-week after payday, the honest answer is that your prescriber will advise, and a short hold at 5mg is usually preferable to rushing a step-up before you are ready.

The Mounjaro 5mg pen page has more on the specific pen format and what to expect when you collect or receive your first 5mg supply. For a broader look at how treatment options compare, our weight-loss overview covers the landscape clearly.
