It happens more often than people expect. You unpack your delivery, get pulled away, and an hour or two later you notice the pen is still sitting where you left it. Or you find an unopened pen in a bag from a weekend away and genuinely can't remember when it came out of the cool. A question our prescribers hear most weeks goes something like: "I left my pen out overnight, is it still okay?"
The first thing to do is locate your Patient Information Leaflet. Novo Nordisk publishes a permitted out-of-fridge duration for unopened Wegovy pens, and that figure (alongside the maximum temperature) is the only number that should guide your decision. The Wegovy SmPC on the electronic Medicines Compendium holds the most current approved storage information; the leaflet inside your box should match it. Do not rely on general internet figures: manufacturers occasionally update these parameters, and the version in your box is what applies to your pen.
What you will not see is any visible sign of degradation. Semaglutide solution stays clear whether the active molecule is intact or partly broken down, there is no colour change, cloudiness or smell to warn you. That is precisely why the time-and-temperature rule exists, and why guessing is the wrong approach.
Semaglutide is a peptide (a chain of amino acids) and peptides are sensitive to heat in a way that small-molecule medicines are not. Above the recommended storage temperature, or beyond the permitted duration, the molecule can begin to degrade: bonds break or the shape of the protein changes in ways that reduce how well it binds to GLP-1 receptors in the body.
The practical consequence is that a degraded pen may deliver less active medicine per dose than the label states, without any obvious outward sign that anything is wrong. Room temperature in a British home or car can vary considerably, a warm kitchen in summer, a car parked in the sun, a bathroom cabinet near a radiator. The permitted out-of-fridge window assumes normal indoor conditions, typically up to around 25°C; sustained exposure above that shortens any safe window further still. If the location where you found the pen was particularly warm, factor that in when you make your assessment.
If the pen has been out of the fridge within the manufacturer's permitted window and at an appropriate temperature, it can generally still be used, confirm this against your leaflet before proceeding. If you are confident the window has been exceeded, the pen should not be injected. Wasted medicine is frustrating, but the alternative is a dose that may be substantially less effective, which could disrupt your titration schedule in ways that are harder to unpick later.
Contact your pharmacy or prescriber. They can advise on replacement and, where relevant, on how to time your next injection so your schedule stays on track. If your pen needs to be disposed of safely, a proper sharps container is the right way to handle it even for an unused pen, since the needle-cap mechanism is already engaged. Do not put the pen in household recycling or general waste.

A few practical habits make accidental out-of-fridge exposure much less likely. Keep your current pen in a consistent spot in the fridge, not the freezer compartment and not pressed against the back wall where temperatures can dip below zero. Freezing inactivates the medicine and a frozen pen should not be used even after it thaws.
When pens are delivered, refrigerate them as soon as possible after the DPD parcel arrives. If you are travelling, a small insulated cool pouch (not direct contact with an ice pack) keeps the pen within range for the permitted duration.
